Get Access To All JobsTips for Finding H-1B Visa Sponsorship as a Hospitalist Physician
Verify your ECFMG certification before applying
Internationally trained physicians need active ECFMG certification before any employer can file an H-1B petition. Confirm your certification status and state medical license are both current so you don't delay an employer's petition timeline.
Target hospital systems with J-1 waiver history
Hospital systems that have previously sponsored J-1 visa Conrad 30 waivers are operationally comfortable with physician immigration filings. These employers already have immigration counsel on retainer and understand the specialty occupation documentation required for hospitalists.
Use Migrate Mate to find verified H-1B sponsors
Search Migrate Mate to filter hospitalist roles by employers with confirmed H-1B LCA filing history. This cuts out the guesswork of cold-applying to health systems that have never sponsored a physician visa.
Request premium processing when your start date is fixed
Hospitalist contracts often specify a credentialing start date tied to hospital privileges. Ask your employer to file Form I-907 for premium processing so USCIS adjudicates the petition within 15 business days and you don't lose your onboarding slot.
Confirm your offered wage meets DOL prevailing wage
Before signing an offer letter, cross-check the offered compensation against the Level II or Level III wage for your SOC code and geographic area using the OFLC Wage Search. Your employer's LCA must certify at or above that figure.
Document nocturnist and subspecialty duties separately
If your role includes nocturnist coverage or subspecialty consultation, ensure your employer's H-1B petition describes each duty component. USCIS scrutinizes mixed-duty hospitalist positions, so a detailed specialty occupation letter tied to your training prevents unnecessary Requests for Evidence.
H-1B Visa Hospitalist Physician: Frequently Asked Questions
Does a hospitalist physician role qualify as a specialty occupation for H-1B purposes?
Yes. Hospitalist physician positions require a Doctor of Medicine or Doctor of Osteopathic Medicine degree at minimum, which satisfies USCIS's specialty occupation standard. The role's reliance on specialized clinical knowledge in internal medicine or family medicine makes it one of the more straightforwardly approvable H-1B categories, though employers must still document the degree requirement in detail.
How do I find hospital employers actively sponsoring H-1B visas for hospitalists?
Migrate Mate filters hospitalist openings by employers with verified H-1B LCA filing history, so you can focus on health systems that have already committed to physician sponsorship. Large academic medical centers, regional hospital networks, and physician staffing groups are the most consistent H-1B sponsors for this specialty.
Can a hospitalist physician transfer an H-1B to a new employer mid-contract?
Yes, through H-1B portability. Once USCIS receives your new employer's H-1B transfer petition, you can begin working for the new hospital without waiting for final approval, as long as your prior status was lawfully maintained. Your new employer files a new Form I-129, and the process typically takes a few weeks to several months depending on whether premium processing is used.
What happens to my H-1B status if the hospital reduces my hours or changes my duties?
Material changes to your role, including significant reductions in hours or a shift to duties outside your approved specialty, can require an amended H-1B petition. Notify your employer's immigration counsel promptly if your contract changes. USCIS considers an amendment necessary any time the terms of employment differ substantially from what was approved in the original petition.
Do hospital employers typically sponsor H-1B visas for locum tenens or per diem hospitalists?
It's structurally difficult. H-1B requires an employer-employee relationship with the petitioner exercising control over the worker's duties, schedule, and supervision. Per diem and locum arrangements often involve staffing intermediaries, which complicates that relationship. Permanent or long-term contract hospitalist positions at a single facility are far easier for employers to petition under H-1B than short-term rotational placements.
