Introduction to the Role and Practice
Centra’s PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community. The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—most often in the comfort and safety of their own home. Our home-based care approach allows participants to maintain independence while receiving comprehensive medical, social, and supportive services.
PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. In this home health–focused model, providers conduct in-home visits, assess living environments, address safety concerns, and coordinate care transitions to reduce hospitalizations and support aging in place. Everything is coordinated with a dedicated interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, home health aides, activities coordinators, and transport staff to ensure seamless, wraparound care.
Centra’s PACE program is seeking a dedicated Advanced Practice Provider to join our team and help shape the future of our home-based care model. This is a unique opportunity to deliver comprehensive, relationship-centered care directly in participants’ homes and community settings. You will play a vital role in chronic disease management, medication reconciliation, preventive care, palliative support, and care plan oversight—meeting patients where they are physically and clinically.

Centra Medical Group (CMG) is a comprehensive network of nearly 600 employed providers within Centra Health. You Can Expect:
- Monday – Friday 8:00am – 5:00pm – no weekends or holidays!
- Primary focus on in-home visits, care coordination, and transitional care management
- Dedicated interdisciplinary team including providers, support staff, physical therapists, occupational therapists, dieticians, social workers, and home health professionals
- 5 patients per day – allowing time for comprehensive home assessments and meaningful patient interaction
- Call is once every 7 weeks, 5pm – 8am, you are not expected to go out, calls are triaged before reaching you!
- This opportunity is perfect for someone looking to build meaningful relationships while delivering hands-on care in patients’ homes and supporting aging in place.
Required Education: Graduate of an accredited Nursing Program, and Masters of Science Degree in Nursing (MSN)
Required Certifications and Licensures: Current licensure by the Virginia State Board of Nursing as a Nurse Practitioner with prescriptive authority. Maintains BLS and ACLS Certifications. Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse. Nationally recognized certification in field of specialization.
