Verify your CGFNS certification before applying
Most U.S. host employers require CGFNS certification or equivalent credential evaluation before they'll engage a J-1 Trainee application. Completing this before outreach shortens the hiring timeline and signals readiness to both the host and designated sponsor.
Target hospitals with existing J-1 host agreements
Not every hospital has an active host organization agreement with a State Department-designated sponsor. Ask the recruiter directly whether the facility has hosted J-1 exchange visitors before, since establishing a new host agreement adds weeks to the process.
Search for J-1 compatible ED roles on Migrate Mate
Use Migrate Mate to filter Emergency Department nursing roles by employers who have demonstrated openness to international exchange visitors, so you spend outreach time on facilities already familiar with the J-1 host process.
Confirm which J-1 category fits your career stage
Nurses within five years of completing their initial nursing degree typically qualify under the Trainee category, while experienced nurses pursuing advanced clinical research programs may qualify as Research Scholars. Your designated sponsor, not the employer, makes the final category determination.
Get your training plan counter-signed before submitting the DS-2019
The J-1 Trainee program requires a detailed Training Plan signed by both the host supervisor and the designated sponsor before the DS-2019 is issued. ED rotations, shift structure, and learning objectives must be documented in the plan to avoid delays during sponsor review.
Check the two-year home residency requirement before accepting an offer
Many J-1 nurses are subject to the two-year home residency requirement based on their country of origin or funding source. Confirm this with your designated sponsor before accepting an offer, since it affects your ability to switch to H-1B or apply for a green card directly from J-1 status.

Find Emergency Department Registered Nurse JobsEmergency Department Registered Nurse J-1 Visa: Frequently Asked Questions
Which J-1 program category applies to Emergency Department Registered Nurses?
Most internationally trained ED nurses enter under the J-1 Trainee category, which covers professionals who have completed their degree within the past five years and are gaining practical clinical experience. Nurses pursuing structured research or advanced clinical fellowships may qualify under the Research Scholar category instead. Your State Department-designated sponsor, such as CIEE or Cultural Vistas, determines the appropriate category based on your background and program design.
Who is actually the J-1 visa sponsor for a nursing role, the hospital or a separate organization?
The hospital is your host employer, not your visa sponsor. The visa sponsor is a U.S. Department of State-designated organization, such as IIE, CIEE, or Cultural Vistas, that issues your DS-2019 form, monitors your program compliance, and signs your training plan. The hospital must have an active host agreement with a designated sponsor before you can proceed, and it's your responsibility to confirm that relationship exists before accepting an offer.
Does the two-year home residency requirement apply to J-1 nurses?
It depends on your country of nationality, your country of residence, and whether your exchange program receives government financing. Nurses from countries on the State Department's Exchange Visitor Skills List, which includes many countries with nursing shortages, are typically subject to the requirement. This means you must return home for two years before changing to most other nonimmigrant statuses or applying for a green card, unless you receive a waiver.
How do I find U.S. hospitals that are open to hosting J-1 Exchange Visitor nurses?
Use Migrate Mate to search for Emergency Department nursing roles at employers who have demonstrated experience with international exchange visitors. Once you identify target hospitals, ask the recruiter or HR contact whether they have an established host agreement with a designated J-1 sponsor organization, since facilities without existing agreements will need additional lead time to set one up before your DS-2019 can be issued.
Can I work in any ED unit on a J-1, or does the training plan lock me into specific duties?
Your J-1 Trainee program is defined by the training plan submitted to your designated sponsor before the DS-2019 is issued. The plan specifies which clinical areas, rotations, and skills you'll develop, and you're generally expected to stay within those defined activities. If your host employer wants to significantly change your role or unit assignment during the program, the training plan typically needs to be amended and re-approved by the designated sponsor.
