Verify your ECFMG certification status early
ECFMG certification is a prerequisite for J-1 Physician sponsorship. Confirm your USMLE scores, medical degree verification, and English proficiency documentation are complete before applying to residency or fellowship programs that host J-1 physicians.
Research Conrad 30 waiver eligibility upfront
The two-year home residency requirement applies to most J-1 Physicians, but the Conrad 30 waiver lets you stay in the U.S. by committing to underserved area practice. Identify states with open Conrad slots early, as each state is limited to 30 waivers annually.
Target NHSC and HPSA-designated host sites
Family practice positions at Health Professional Shortage Area facilities commonly sponsor J-1 Physicians and often support Conrad 30 waiver applications simultaneously. HRSA's shortage area database lists qualifying sites by state and county.
Search J-1-aligned family medicine roles on Migrate Mate
Use Migrate Mate to filter family practice physician openings at U.S. host employers that align with J-1 Physician program requirements, so you're targeting sites already familiar with ECFMG-administered sponsorship rather than educating every recruiter from scratch.
Confirm the training plan matches your program objectives
ECFMG requires a detailed training plan before issuing your DS-2019. Work with your host program coordinator to ensure the plan reflects your clinical objectives, supervision structure, and evaluation schedule, since mismatches can delay or invalidate your sponsorship.
Clarify prevailing wage compliance with your host employer
J-1 Physician positions must meet DOL wage standards. Cross-reference your offered compensation against OFLC Wage Search data for the relevant SOC code and geographic area before signing your contract to avoid compliance issues during your program.

Find Family Practice Physician JobsFamily Practice Physician J-1 Visa: Frequently Asked Questions
Which J-1 program category applies to Family Practice Physicians?
Family Practice Physicians entering U.S. residency or fellowship programs use the J-1 Physician category, not the Intern or Trainee category. ECFMG is the sole State Department-designated sponsor for this category. ECFMG issues your DS-2019, monitors program compliance, and liaises with your host clinical site throughout your training period.
Does the two-year home residency requirement apply to J-1 Family Practice Physicians?
Yes. Most J-1 Physicians are subject to the two-year home residency requirement under INA Section 212(e), meaning you must return to your home country for two years after your program before adjusting to H-1B or permanent resident status. The Conrad 30 waiver and Interested Government Agency waivers are the primary paths to avoiding this requirement for physicians who commit to underserved area practice.
Is the hiring hospital or clinic my J-1 visa sponsor?
No. In J-1 physician programs, the hiring hospital or clinical site is your host organization, not your visa sponsor. ECFMG is the designated sponsor that issues your DS-2019, tracks your compliance obligations, and maintains your SEVIS record. The host employer provides your clinical training environment and compensation but has no direct role in sponsoring the visa itself.
How do I find family medicine positions where the host employer already understands J-1 Physician requirements?
Migrate Mate lets you search family practice physician openings filtered by J-1 compatibility, helping you identify U.S. host employers that are already familiar with ECFMG sponsorship, training plan requirements, and Conrad 30 waiver processes. That familiarity significantly reduces the back-and-forth during the offer and onboarding stage.
Can a J-1 Family Practice Physician work in private practice rather than a hospital system?
Yes, but the host site must meet ECFMG's program criteria, including having a qualified supervising physician, an approved training plan, and the infrastructure to support ECFMG monitoring. Smaller private practices can qualify, but they must demonstrate these requirements before ECFMG will issue a DS-2019. Confirm this readiness with the practice administrator before accepting any offer.
