Verify your CGFNS certification before applying
Most U.S. host hospitals require Commission on Graduates of Foreign Nursing Schools (CGFNS) certification and a state nursing license before they'll engage with a J-1 sponsor on your behalf. Start this process six to twelve months ahead.
Identify host employers with active J-1 programs
Search Migrate Mate to find hospitals and health systems that have hosted J-1 exchange visitors in nursing roles. Not every employer knows how to work with a designated sponsor, so targeting those with prior J-1 experience saves significant time.
Distinguish the Trainee and Research Scholar categories
The Trainee category fits nurses with a degree and at least one year of post-graduation experience who are seeking structured clinical training. Research Scholar applies if your placement centers on nursing research at an academic medical center rather than direct patient care.
Confirm your host employer will sign the training plan
Your designated sponsor, such as CIEE or Cultural Vistas, requires a signed Training or Exchange Visitor Placement Plan before issuing your DS-2019. Secure written commitment from your host employer's HR team early, since delays here stall the entire sponsorship process.
Check whether the two-year home residency requirement applies
Nurses sponsored by government-funded programs or coming from countries on the Exchange Visitor Skills List are subject to a two-year home residency requirement before changing to H-1B or permanent residence status. Confirm your program funding source with your designated sponsor before accepting an offer.
Align your clinical specialty with your training objectives
Designated sponsors and USCIS scrutinize whether your proposed training in, for example, ICU or NICU nursing is genuinely unavailable in your home country. Document the gap clearly in your Training Plan to avoid a Request for Evidence from USCIS.

Which J-1 program category applies to Registered Nurses?
Most internationally trained Registered Nurses enter the United States under the J-1 Trainee category, which requires a nursing degree and at least one year of post-graduation clinical experience. If your placement is at an academic medical center focused on nursing research rather than direct patient care, the Research Scholar category may apply instead. Your designated sponsor determines the correct category based on your background and the host employer's program.
Who actually sponsors my J-1 visa as a nurse?
Your J-1 visa sponsor is a U.S. Department of State-designated organization, not your hospital employer. Organizations such as CIEE, Cultural Vistas, and similar entities issue your DS-2019, monitor your program compliance, and maintain your SEVIS record. The hospital or health system where you work is your host employer, not your visa sponsor. These two roles are legally distinct, and confusing them can cause serious delays in your application.
Does the two-year home residency requirement affect nurses?
Yes, it can. If your J-1 program receives U.S. government funding or your home country appears on the Department of State's Exchange Visitor Skills List for nursing, you'll be subject to the two-year home residency requirement. This means you must return home for two years before applying for an H-1B or green card, unless you obtain a waiver. Confirm your status with your designated sponsor before signing any offer.
How do I find U.S. hospitals that host J-1 nurses?
Use Migrate Mate to search for hospitals and health systems that have hosted J-1 exchange visitors in nursing roles. Not all hospitals are familiar with the designated sponsor model or willing to sign the required Training Plan, so identifying employers with prior J-1 hosting experience significantly shortens your search and improves your chances of reaching an offer stage.
What credentials do I need before a host employer will engage?
Most host hospitals require CGFNS certification, a valid state nursing license or eligibility letter, and English proficiency test results such as IELTS or TOEFL before they'll begin coordinating with a designated sponsor. Some states also require NCLEX passage before licensure. Starting credential verification six to twelve months before your intended start date gives you enough runway to meet these requirements without delaying your DS-2019 issuance.
