SUMMARY
Under the supervision of the Operations Manager, the Case Manager - Licensed Clinical Social Worker (LCSW) provides direct case management to veterans residing, at Veterans Helping Veterans Heal (VHVH) Expansion and offers clinical supervision to other case managers, social workers, and interns. This role ensures that each veteran’s individual needs are met using best practices, evidence-based interventions, and a trauma-informed approach, while guiding residents toward permanent housing, stable employment, and independent living. The position requires flexibility to work nonstandardhours, including evenings, weekends, and participation in an on-call rotation, to meet program and resident needs. This role has the potential for expansion and growth based on performance and integration into the role.
The VHVH Thirty-Seven bed extended-term housing program in Winston-Salem, NC for veterans battling homelessness to ultimately transition into permanent housing and independent living. VHVH is a partner in the 10-Year Plan to End Chronic Homelessness and serves homeless veterans with mental health and/or substance abuse issues referred by the Veterans Administration (VA).
The mission of VHVH is to see homeless veterans shift from being net-consumers of community services to net-contributors to community resources. To accomplish our mission VHVH provides veterans safe and secure housing, professional case management services on site, and a new supportive community.
DUTIES AND RESPONSIBILITIES
These describe the general nature of the position; other duties and responsibilities may be assigned as needed.
- Provide clinical supervision to case managers, social workers, and interns in compliance with NC LCSW requirements.
- Maintain a caseload of veterans in the 90/90/360/90 program model.
- Develop and oversee person-centered service plans addressing six core goal areas: housing/financial, substance use, legal, mental/physical health, employment/education, and social engagement.
- Serve as a clinical resource for crisis intervention, de-escalation, and trauma-informed care.
- Facilitate and/or co-facilitate life skills classes, daily programming, and community meetings.
- Coordinate with VA, NCWorks, Goodwill, and other employment and service partners.
- Advocate for residents and make referrals to community-based programs.
- Ensure timely and accurate documentation in paper files and electronic databases (HMIS, Outcome Tracker).
- Assist with operational tasks including transportation coordination, VA GPD billing, resident budget support, service fee collection, and donation tracking.
- Support property management needs and participate in facility tours.
