DESCRIPTION
We are seeking a licensed and dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing team. The provider will travel to skilled nursing facilities across New York to perform monthly patient visits.
This is an excellent opportunity for a motivated clinician who enjoys working independently, making a direct impact on patient care, and maintaining flexibility in their schedule.
Responsibilities:
Conduct monthly visits on assigned patients in skilled nursing facilities
Review patient charts, document findings, and update care plans as needed
Collaborate with facility staff to ensure continuity of care
Maintain compliance with all state, federal, and company documentation standards
Requirements:
Active New York NP or PA license (required)
Experience in geriatrics, long-term care, or internal medicine preferred
Willingness to travel to multiple facilities across North Jersey
Strong communication and clinical documentation skills
Reliable transportation
