Clinical Coordinator RN
Dialysis Experience Required
Make a difference in the lives of patients with chronic kidney disease.
The Clinical Coordinator RN will work on-site at DaVita's 6-chair Skilled Nursing Facility dialysis dens. The modality eliminates the need for patient travel to an outpatient dialysis facility. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a burden on patients and adding avoidable costs to the health care system.
DaVita is seeking a Clinical Coordinator RN to lead dialysis care in a skilled nursing facility. In this role, you’ll oversee patient care delivery, support a clinical team, and ensure treatment meets the highest standards of safety and quality.
Key Responsibilities:
- Coordinate care for ESRD patients, including labs, vitals, and outcomes
- Lead and support a team of clinicians and PCTs
- Deliver direct patient care in a skilled nursing facility
- Monitor patient status and adjust care as needed
- Ensure compliance with all clinical and regulatory standards
- Support Administrative Growth
- Communication with our SNF partner
- Coaching teammates on the resolution of issues and escalating to Regional Operations Manager as appropriate
- Staffing the floor
- Administrative duties

Qualifications
- RN license in the state of practice
- Minimum of 18 months of RN experience
- Minimum 6 months of dialysis experience
- CPR certification
- Charge RN readiness approval required
- CNN or CDN certification preferred
- Supervisory experience preferred
- Proficiency in MS Word and Outlook
