Responsibilities
- Evaluate and treat patients in their homes to improve safety, independence, and functional performance with daily activities
- Develop individualized treatment plans and home exercise programs
- Assess and recommend adaptive equipment, home modifications, and fall-prevention strategies
- Collaborate with physicians, nurses, PTs, and other healthcare professionals
- Educate patients and caregivers on functional mobility, self-care, and safety techniques
- Complete timely and accurate documentation in Kinnser/WellSky
Pay: $80.00 - $110.00 per hour
