Physical Therapist
Location: Rutland, Vermont
Pay Rate: $2,200 to $2,300/week
Contract: 13 weeks
Schedule: Monday Friday, 8:00 AM 4:00 PM; 2 3 weekend days required
Guaranteed Hours: 37 hours per week
Experience: Minimum 2 years required
About the Role
We are seeking an experienced Home Health Physical Therapist for a 13-week contract opportunity covering Rutland and Bennington County, Vermont. This role provides skilled physical therapy evaluations, OASIS assessments, treatment planning, patient education, care coordination, and home-based rehabilitation services.
Responsibilities
- Perform comprehensive PT and OASIS assessments in the home health setting
- Develop individualized plans of care to improve function, safety, and independence
- Provide skilled physical therapy treatments, patient education, and caregiver support
- Coordinate care with physicians, nurses, rehab teams, caregivers, and schedulers
- Document visits accurately using McKesson and complete required home health paperwork
Requirements
- Active Vermont Physical Therapist License Required Upon Acceptance
- Minimum 2 years of Physical Therapist experience required
- Home Health experience preferred
- OASIS and McKesson experience preferred
- Must be available for Monday Friday schedule with 2 3 weekend days and coverage across Rutland and Bennington County
