General Summary
Provides direct patient care and drug therapy management services for patients with specialty conditions upon provider referral. Working under a written protocol, the pharmacist partners with providers to manage a patient's drug therapy, optimize outcomes, improve access to specialty medications, and ensure safe, cost-effective medication use.

Remote Work Capable
Essential Functions:
- Evaluates appropriateness of drug therapy for patients with specialty conditions referred to the WellSpan Management of Drug Therapy (MDT) Clinic (e.g., rheumatology, dermatology, endocrinology).
- Adjusts drug therapy and order laboratory tests or other diagnostic tests necessary in the management of drug therapy under Collaborative Practice Agreement (CPA) or written protocol in accordance with state law.
- Monitors treatment efficacy, therapeutic outcomes, safety parameters, and adherence for patients receiving biologics and other specialty agents.
- Identifies and resolves drug therapy problems throughout the course of medication therapy.
- Provides patient education and counseling regarding disease state, medication use, side effects, and lifestyle modifications.
- Communicates and documents therapy changes and updates with providers in accordance with written protocol in the electronic health record (EHR).
- Collaborates with providers, nurses, and other care team members in the different specialty practices or pharmacy to coordinate care.
- Monitors adherence and persistence for patients receiving specialty medications; implement adherence strategies when gaps are identified.
- Ensures compliance with accreditation standards (URAC, ACHC, or NCQA if applicable), and organizational policies.
- Participates in quality improvement, program development, accreditation compliance activities, and payor contracting guidelines.

- Stays current with evidence-based practice, specialty guidelines, and regulatory updates through continuing education, seminars, workshops, and professional affiliations.
- Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
- Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
Qualifications
Minimum Education:
- Bachelors Degree Bachelor of Science in Pharmacy from an accredited College of Pharmacy. Required or
- Doctorate Doctorate of Pharmacy Degree (PharmD) from an accredited College of Pharmacy. Required
Work Experience:
- 3 years Relevant experience. Required or
- 1 year Completion of an ASHP-accredited PGY-1 residency program. Required
Licenses:
- Licensed Registered Pharmacist Upon Hire Required
Courses and Training:
- Completion of a pharmacy program from an accredited school of pharmacy. Upon Hire Required

How does TN visa compare to H-1B for Clinical Pharmacists?
TN is faster and more predictable for pharmacists who are Canadian or Mexican citizens.
