INTRODUCTION
A Clinical Pharmacist – Multisite contributes to AHF by expanding primary care functions of pharmacists at more than one site to provide individualized health promotion and disease prevention, including administration of vaccines, long-acting HIV treatments, diabetes monitoring, nicotine reduction programs and other subject matters, in collaboration with other AHF providers.
A Clinical Pharmacist – Multisite enhances the reputation of AHF by providing valuable contributions to more than one location by bringing “best practice” and collaborative ideas, as well as providing consistent application of processes, procedures and policy throughout the organization.

The Clinical Pharmacist – Multisite will work as a part of a close-knit team to provide individualized health promotion and disease prevention, including administration of immunizations, diabetes and smoking cessation programs, as well as other Subject Matter Expertise (SME) programs as developed by AHF, with appropriate collaborative drug therapy management authority. This includes prescriptive authority where this is legally and organizationally authorized such as PrEP and PEP therapies.
