Primary Purpose:
The person in this position is a member of the Patient Centered Health Care Home team where each team member plays an integral role in providing patient-centered health care. S/he provides triage services telephonically and personally by assessing patients for urgency of medical intervention then coordinating their access to care.
Essential Duties:
Triage Function
- Triage calls for same day appointments for patients when none is available, based upon predetermined protocols.
- Assess and establish medical urgency and language needs of walk in patients desiring same day access.
- Provide health advice and information telephonically or personally to patients based on existing protocols and/or as directed by a provider.
- Provide limited, short-term case management/care coordination services including but not limited to and follow-up for laboratory & diagnostic testing, pharmacy, consultants and health care agencies.
- Assist patients who need medication refills. This includes receiving request for refills, calling in prescriptions and triaging calls from pharmacist regarding medications.
- Provides patient education as necessary.
- Performs floor duties as necessary such as checking in and checking out of patients.
- Informs patients on abnormal laboratory results.
Administrative Responsibilities
- Support and supervise medical support staff working with him or her.
- Participate in the development of appropriate triage policies and procedures.
- Maintain an awareness of services offered through the clinic and in the community to serve client needs.
- Maintain awareness of legal and legislative issues that may impact service availability.
- Participate in clinic quality improvements activities.
- Work in concert with different disciplines to assess triage strategies.
- Maintain client confidentiality per clinic protocol.
- Develop and implement forms for data collection, tracking and documentation of activities and work with different teams to determine effective communication strategies.
- Work closely with other clinic staff engaged in activities or projects involving clients as relates to patient care.
Regular and reliable on-site attendance is required.
Other Duties:
Perform other related duties as assigned.
- Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.
Minimum Qualification Requirements:
Skills/Knowledge:
- Knowledge and skill in the delivery of community health nursing care.
- Basic computer literacy, with functional understanding of Windows 95/98 and Microsoft Word. Database creation and maintenance also desirable.
Education:
- Bachelor of Science in Nursing preferred.
- Current State of Hawaii LPN license required.
- Current State of Hawaii RN license preferred.
- Current CPR Certification required.
Experience:
- Clinical experience working with communities and providing community health education outreach preferred.
- Comfortable working in managed care/multi-professional, multi-cultural environment.
- Knowledge of community resources, how to access such, and an awareness of government assistance programs.
- Ability to assess and plan in a multi-cultural context.
- Professionalism, sound clinical judgment and skills and emotional stability.
