Palliative care is specialized medical care for people living with a serious, progressive, often life limiting illness. This type of care is focused on providing relief from the symptoms and stress of the illness, to improve quality of life for both the patient and the family. Examples of serious illness often seen by a palliative care team include all types of cancer, end stage organ disease (e.g., heart, lung, liver, kidney), dementia, Parkinson’s disease, and other progressive neurological disorders.
The Palliative Care Social Worker is part of a multidisciplinary team that partners with patients to provide consultative care in the inpatient setting. They work alongside an expert palliative care provider and other medical specialists to complete assessments, facilitate goals of care discussions and advance care planning conversations, coordinate care with other members of the patient’s treatment team, provide information and referrals to community resources, and provide overall support to patients and their families.
ROLE AND RESPONSIBILITIES
- Works in tandem with a physician or APP providing support via serious illness conversations.
- Serves as a member of a multi-disciplinary team and is a liaison with community resources.
- Provides community resource information and referrals.
- Float coverage for Boise, Meridian and Nampa hospitals.
- Monday-Friday.
- Full Time with benefits and hospital contributions.
- Intensive onboarding and training.
BASIC QUALIFICATIONS
- Education: Masters degree in social work.
- Experience: 0 years’ experience.
- Licenses/Certifications: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in states of practice.
PREFERRED QUALIFICATIONS
- Preferred experience in Hospice or medical field.
