JOB DESCRIPTION:
About the Program:
We’re looking for a proven, people-first leader with experience implementing community health initiatives and managing teams in community-based or nonprofit settings to take on a pivotal leadership role in the success of our Horizon Program.
Key Responsibilities:
- Implement community health initiatives within a rural health service area
- Track and analyze performance data to ensure strong patient outcomes
- Manage project budgets and basic accounting for program expenses
- Supervise and mentor staff
- Facilitate collaboration and linkages to care with clinical care teams and community organizations
- Facilitate team meetings and educational opportunities for patient support groups
The Ideal Candidate Has:
Strongly Preferred:
- Demonstrated experience executing workplans in a nonprofit, healthcare or community-based setting.
- Background in public health, grants management, community programming
- Experience with grant writing and program reporting
Preferred:
- Strong understanding of healthcare, social services, and public benefits systems
- Demonstrated experience managing teams in a nonprofit, healthcare, or community-based setting.
- Familiarity with community resources and networks in Central Maine
- Proficiency in using data to drive decision-making and improve outcomes
- Excellent communication and interpersonal skills
- Bachelor’s degree in Public Health (PH), related field, or equivalent experience preferred.
