INTRODUCTION
The Mental Health Therapist works under the supervision of the Practice Manager to provide clinical assessment, referral and/or counseling for mental health problems.
ROLE AND RESPONSIBILITIES
- Provide assessment, diagnosis, counseling, and follow-up to clients. Maintains and interprets documentation of clinical work and other statistically relevant data; meeting quality assurance standards.
- Provides assessment, counseling and/or referral services to patients at Mosaic Outpatient Behavioral Health or Mosaic Counseling Care.
- Provide computerized/written reports and/or clinical documentation for legal purposes and administrative accountability.
- Attend relevant and required meetings to provide consultation and gain needed information relevant to work.
- Available for clients in emergency situations.
- Complete clinical notes in a timely manner and paperwork necessary for insurance filing.
- Maintain broad expertise in clinical areas and certifications or relevant credentialing.
- Offer in-service education programs and functions as a resource to the medical staff and the community as needed.
- Work cooperatively with Mosaic Life Care providers and personnel to carry out the goals and objectives of Mosaic Life Care. Maintain good communication and collaborative relationships with referring physicians and other staff and recommend additional referrals for patients as needed.
- Provide screening/evaluation of individuals for mental health and substance abuse disorders. Assess, diagnose, and evaluate clinical symptoms, developmental and personality disorders, severity of psychosocial stressors and global functioning and address medical conditions as they affect patients’ functioning. Provide behavioral supports to assist individuals in improving health status and managing chronic illnesses.
- Conduct brief interventions for individuals with behavioral health problems. Provide specific and goal directed interventions: use monitoring forms, use behavioral health “prescription”, implement multiple interventions simultaneously.
- Meet regularly with the care team to plan care and discuss cases, and exchanges appropriate information with team members in an informal “curbside” manner as part of the daily routine of the clinic. Provide feedback to Provider consisting of: clear, concise, brief interaction; focused on referral question; description of action plan; plan for follow-up.
- Provide support to Provider/teams in identifying and behaviorally intervening with patients who could benefit from behavioral intervention. This includes identification of the problem behavior, discussion of impact and deciding what to change.
- Plan treatment based on the individual client’s needs and resources and on clinic and community resources for supplementary treatment which adequately and appropriately utilize these resources. Coordinate professionals involved in patient care to insure interdisciplinary collaboration and continuity of services. Provide a continuum of short- and long-term psychotherapy services to individuals, couples, families, and groups, based on individualized treatment plans.
