Clinical Executive Director – Pediatrics (RN)
Location: Rochelle Park, NJ

Affirmed Home Care, a premier concierge private-pay home care agency, is seeking an experienced Clinical Executive Director (RN) to lead and oversee pediatric clinical operations across New Jersey, New York, and Connecticut.
This is a key leadership opportunity for an experienced RN who is passionate about pediatric home care, clinical excellence, high-acuity care, regulatory compliance, and developing strong nursing teams.
Key Responsibilities
- Lead and oversee pediatric clinical operations across NJ, NY, and CT.
- Supervise and support pediatric RNs and LPNs while providing clinical guidance and real-time problem solving.
- Manage a designated caseload of pediatric clients, including VIP, high-acuity, and high-tech cases.
- Provide hands-on clinical leadership for start-of-care visits, assessments, supervisory visits, and discharges.
- Ensure timely and accurate completion of Plans of Care (485s), physician orders, assessments, visit notes, and other clinical documentation.
- Conduct chart reviews and self-audits to ensure compliance with DOH, Medicare, Medicaid, managed care, and agency standards.
- Oversee insurance authorizations, reauthorizations, Single Case Agreements (SCAs), and Letters of Medical Necessity (LMNs) to support continuity of care.
- Monitor authorization utilization and proactively address renewals, documentation deficiencies, denials, and appeals.
- Participate in QA/QAPI initiatives and identify opportunities to improve clinical quality, documentation, compliance, and reimbursement.
- Support nursing orientation, competency validation, education, coaching, and performance management.
- Collaborate with Billing, Compliance, Intake, Scheduling, HR, physicians, hospitals, and insurance representatives.
- Manage pediatric on-call coverage and help address staffing and clinical needs during off-hours when necessary.
- Support recruitment, evaluation, and development of high-performing pediatric nursing teams.
- Travel regularly between NJ, NY, and CT offices as needed.
Requirements
- Active Registered Nurse NJ (RN) license
- 5+ years of pediatric home care leadership experience preferred
- Strong knowledge of pediatric home care, clinical documentation, and regulatory requirements
- Experience with Medicaid, Medicare, managed care, authorizations, and payer requirements
- Strong leadership, communication, organization, and problem-solving skills
- Ability to manage multiple priorities in a fast-paced environment
- Willingness to travel between NJ, NY, and CT
- Flexibility to support occasional evenings and weekends as needed
