By integrating primary care, mental health, gynecology, and wellness across both in-person and virtual settings, Tia is improving outcomes, lowering costs, and creating a better experience for patients and providers alike.
About the Role
This is an ideal opportunity for a clinician looking to expand their leadership scope without leaving the bedside. We are seeking a Medical Director (MD/DO) to lead our Williamsburg & SoHo clinics. In this role, you'll partner closely with operations to mentor your team, champion clinical excellence, and shape a world-class experience for both patients and providers.
The "Player-Coach" Model: To remain deeply rooted in our mission, this is a patient-facing leadership position. You will spend approximately 95% of your time in direct clinical care, leading by example and using your frontline insights to drive team development and continuous improvement.
Why Tia
- Built-in protected time for documentation, care coordination, and team collaboration, including dedicated weekly management time aligned across markets
- AI-supported documentation to reduce charting burden
- Beautiful, thoughtfully designed clinics in prime locations
- Quarterly bonus + equity not typical in clinical roles
- Opportunity to build and shape a new model for women's healthcare
Clinical Leadership & Care (95%)
- Lead by Example: Provide high-quality, evidence-based patient care, setting the clinical standard for your team and acting as a model for Tia's care philosophy.
- Master Clinical Quality: Own clinical outcomes and documentation consistency within your clinic, ensuring your team delivers care that is both high-touch and high-quality.
- Manage Complexity: Act as the clinical point person for complex patient cases or team issues, utilizing sound judgment and professionalism to resolve challenges in real-time.
Clinic Operations & Development (5%)
- Drive Team Performance: Partner closely with operations to lead and develop a high-performing provider team, driving engagement, accountability, and a shared commitment to excellence.
- Optimize the Experience: Collaborate on operational workflows to ensure a seamless, "stronger-together" experience for both patients and the clinical staff.
- Continuous Improvement: Use data-driven insights and patient feedback to identify clinical gaps and implement improvements that elevate the standard of care at Silver Lake.
- Build the Team: Support the onboarding and ongoing mentorship of new providers, fostering a culture of continuous learning and professional growth.
What You'll Bring
Requirements:
- Credentials: MD/DO; Board Certified in FM, IM, or OB/GYN; Active NY License and DEA.
- Clinical Expertise: 3+ years in outpatient primary care with a focus on Women's Health (routine gyn, contraception, STIs), Mental Health, and inclusive, patient-centered care.
- Leadership & Accountability: 1+ years leading clinicians (direct or indirect); proven ability to coach teams, hold staff accountable, and perform peer reviews.
- Operational Mindset: Data-driven and adaptable; comfortable collaborating with Operations to balance clinical quality with workflow efficiency.
Training & Development
Candidates who lack experience in the following areas will receive specialized training:
- Specialized Women's Health: Menopause management and sexual health.
- Expanded Primary Care: Medical dermatology, weight management, and procedural scope.
Work Location
This role is based in our Williamsburg & SoHo clinics.
Ownership of Your Schedule: As the leader of our Williamsburg & SoHo clinics, you'll have the flexibility to determine your site rotation. We prioritize clinical outcomes over rigid scheduling and will partner with you to create a split that works for you and the teams you lead.
Compensation & Benefits
Tia is committed to pay equity and pay transparency. Compensation for this role will be determined based on job-related factors, including experience, skills, qualifications, location, role leveling, business needs, and market conditions.
The expected base salary range for this role is $245,000 - $290,000, depending on level and experience.
- Medical Director Stipend: An additional $8,000 annually.
- + Growth Incentive: This stipend is designed to scale upward as the clinic expands and patient volume increases.
You are also eligible for:
- Quarterly performance bonus (10% target)
- Equity grants
- Medical, dental, vision
- Malpractice coverage
- Paid time off (PTO), sick leave, holidays
- CME stipend and licensing reimbursement
- Access to clinical tools and ongoing training resources

Which companies sponsor visas for medical directors in New York?
Large health systems are the most active sponsors for medical directors in New York. NYC Health + Hospitals, Northwell Health, Memorial Sloan Kettering Cancer Center, Mount Sinai Health System, and NewYork-Presbyterian are among the employers with established immigration programs. Health insurance organizations like EmblemHealth and MetroPlus Health also hire medical directors and have sponsored work visas for qualified physicians.
Which visa types are most common for medical director roles in New York?
The H-1B visa is the most common visa for medical directors in New York, as the role typically qualifies as a specialty occupation requiring a medical degree. Physicians with exceptional research or clinical records may qualify for the O-1A. Those pursuing permanent residence often pursue EB-1B (outstanding researcher) or EB-2 NIW pathways, both of which are well-suited to senior clinical and academic roles.
Which cities in New York have the most medical director sponsorship jobs?
New York City accounts for the large majority of medical director sponsorship opportunities in the state, given its concentration of hospital systems, academic medical centers, and managed care organizations. Buffalo and Rochester have meaningful demand through Catholic Health, Kaleida Health, and the University of Rochester Medical Center. Albany sees openings through Albany Medical Center and state-affiliated health organizations.
How to find medical director visa sponsorship jobs in New York?
Migrate Mate filters job listings specifically for roles that offer visa sponsorship, making it straightforward to browse medical director openings in New York without sorting through positions that don't sponsor. You can filter by location and role type to focus on New York health systems, academic medical centers, and health plans that have a track record of sponsoring international physicians for medical director positions.
Are there state-specific considerations for medical directors seeking visa sponsorship in New York?
New York requires medical directors to hold a valid New York State medical license, which international physicians must obtain through the New York State Education Department before starting work. The state's prevailing wage requirements for H-1B positions are determined by the Department of Labor and reflect New York City's high-cost market. Academic medical centers affiliated with Columbia, Cornell, and NYU often have in-house immigration counsel experienced with physician sponsorship.
What is the prevailing wage for sponsored medical director jobs in New York?
U.S. employers sponsoring a visa must pay at least the prevailing wage, which is what workers in the same role, area, and experience level typically earn. The Department of Labor sets this rate to make sure companies aren't hiring foreign workers simply because they'd accept lower pay than a U.S. worker. It varies by job title, location, and experience. You can look up current prevailing wage rates for any occupation and location using the OFLC Wage Search page.
