What You Will Do:
- Clinical Execution & Physician Partnership: Drive clinical excellence by accurately executing complex medical orders and treatment strategies, acting as a high-level partner to physicians in delivering expert, patient-centered care.
- Interdisciplinary Coordination: Serve as the central communication hub for the interdisciplinary team, ensuring seamless collaboration and the delivery of holistic, well-integrated care journeys.
- Patient Advocacy & Health Monitoring: Advocate for optimal outcomes by proactively tracking health trends and documenting progress, ensuring the medical team has the insights needed for timely, life-changing interventions.
- Discharge & Transition Leadership: Lead comprehensive transition planning to empower patients and their families, providing the education and tools necessary for a confident and successful recovery at home.
What You Will Need:
Licensure / Certification / Registration:
- Required Credential(s):
- Advanced Life Support obtained within 6 Months (180 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- Licensed Registered Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date.
Education:
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
