Gifford Health Care is seeking a Clinical Care Manager to join our grant-funded Community Health Team. In this role, you will support patients with complex and chronic health conditions by providing care management, education, and care coordination services that improve health outcomes and strengthen self-management skills.
A strong foundation in chronic disease management is essential, including knowledge of conditions such as diabetes, heart failure, cardiovascular disease, COPD, and other complex medical concerns. The Clinical Care Manager assesses patient needs, readiness for change, and understanding of their health conditions, then delivers individualized education and coaching using evidence-based approaches to promote effective disease management and informed decision-making. This role requires exceptional skill in communicating complex health information in ways that are understandable, actionable, and meaningful for patients and caregivers with diverse health literacy levels, learning styles, cultural backgrounds, cognitive abilities, and support systems. Through personalized teaching, motivational interviewing, and ongoing support, the Clinical Care Manager empowers patients to build confidence, develop self-management strategies, adhere to treatment plans, and achieve their health goals.
As part of the Vermont Blueprint for Health, the Clinical Care Manager contributes to care coordination, chronic disease management, prevention, and population health initiatives while maintaining patient confidentiality and compliance with all organizational and regulatory requirements.

- Current Vermont license as a LPN, RN, OT, OTA. PT, or PTA
