ABOUT THE JOB
Fostering Connections provides services and supports for young people from ages 16.5-26 to include assistance with job applications, college applications, support with Medicaid and Snap and any other services needed while enrolled in Fostering Connections. Fostering Connections assists with different funding sources to assist with educational training and funding to support with furniture, deposits for housing and other resources to assist young people to transition successfully into adulthood.

PURPOSE:
This position exists to utilize data to determine compliance with federal and state regulations and requirements (e.g. Chafee, ETV, Families First, etc.) related to Older Youth Services. The incumbent coordinates communication and reporting regarding federal compliance in relationship to older youth services with internal partners and state/federal reporting entities. This position provides technical assistance to OYS staff, internal partners and others regarding federal requirements and efforts towards 100% compliance in providing services and supports for youth transitioning from foster care to adulthood; the incumbent will partner with public/private agencies that serve transition-aged youth and receive federal pass through funding (IV-E/Chafee) to strengthen these relationships and improve access to services and supports.
Responsibilities:

Serve as primary contact for Fostering Connections regarding federal compliance and the Annual Progress and Services Report (APSR) and CFSR by gathering required data and reporting elements from internal and external partners. 30%

Coordinates, develops and reviews draft(s) of the APSR five-year plan and annual updates with the Program Manager and FC Field/Program Associate Deputy Directors. 10%

Serve as the primary contact for the Credit Reporting Bureaus and run and resolve youth credit reports as identified in policy and procedure and in compliance with federal standards. 15%

Serve as the primary contact for Start Up Applications. Review, process and submit Start Up applications to ensure applications are complete and processed in a timely manner. 15%

Serve as the primary contact for Tribal and Interstate Referrals. Review, process and coordinate enrollment and connection for Fostering Connections services for referrals received through tribal and interstate sources. 10%

Serve as the primary point of contact for NYTD surveys and reports. Work with Fostering Connections specialists to ensure that NYTD surveys are completed in a timely manner. 10%

Research state and national regulations and practices as related to innovative services and support that are in compliance with federal regulations (Chaffee, IV-E, etc.) and provide innovative programming ideas and recommendations to Fostering Connections leadership. 5%
BASIC QUALIFICATIONS:
High School diploma or equivalent and two (2) years of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling two (2) years may substitute for the required experience.
EMPLOYMENT REQUIREMENTS:
Background investigation is required and is conditional pending results.
WORKING CONDITIONS:
Work is performed in an office setting, exposure to Visual/Video Display terminal (VDT) and extensive personal computer and telephone usage with extended periods of sitting. May work under frequent time constraints. Travel will be required.
