Location: Orange, NJ
A Skilled Nursing Facility in Orange, NJ is seeking a licensed Physical Therapist (PT) to provide rehabilitative services to short-term rehabilitation and long-term care residents. This is a full-time, permanent role requiring prior experience in a Skilled Nursing or post-acute setting.
The Physical Therapist will work collaboratively within an interdisciplinary team to support functional improvement, safe discharge planning, and regulatory-compliant documentation.
Key Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans
- Provide therapy services to short-term rehab and long-term care residents
- Monitor patient progress and modify treatment plans as appropriate
- Maintain accurate, compliant documentation in accordance with Medicare and facility standards
- Participate in interdisciplinary care plan meetings and rounds
- Support discharge planning and functional goal achievement
- Ensure compliance with Medicare Part A, Part B, and payer documentation requirements
Required Qualifications
- Active New Jersey Physical Therapist license required
- Minimum 1+ year of experience in a Skilled Nursing Facility or post-acute rehab setting required
- Strong understanding of skilled documentation standards and Medicare guidelines
- Ability to manage a productive caseload in a SNF environment
- Strong communication and interdisciplinary collaboration skills

Does a Physical Therapist CPT role require a state license, and how does that affect my start date?
Yes, most states require full PT licensure before you can practice independently, even under CPT. You'll need to pass the NPTE and apply for your state license separately from your CPT authorization. Your DSO can authorize CPT before licensure is issued, but your employer typically won't let you start treating patients until the license arrives, so build that gap into your timeline.
Can I do CPT at a private outpatient PT clinic, or does it need to be a hospital?
CPT is not limited to hospital settings. Private outpatient clinics, rehabilitation centers, skilled nursing facilities, and home health agencies all qualify as long as your school documents how the work integrates with your curriculum. The key test is whether your DSO can show a direct connection between your role and your enrolled coursework, not the employer's size or setting.
