JOB RESPONSIBILITIES
Coordination and Preparation:
- Reports educational progress to patient's school.
- Collaborates with intradisciplinary team on preparation for transition back to school.
- Consults with patient and family regarding plans for re-entry.
- Facilitates the re-entry process following discharge.
- Participates in patient care conferences.
Interaction:
- Meets new patients and gathers pertinent information through observation and interview.
- Assesses educational needs in classroom and at bedside.
- Makes appropriate recommendations based on accurate assessments.
- Makes internal and external referrals as indicated.
Assessment:
- Assesses educational level and cognitive status of patients.
- Identifies physical readiness and cognitive readiness for participation in school program.
Goal Setting:
- Formulates appropriate educational goals.
- Develops curriculum based on individual patient educational goals.
- Implements educational programming based on educational assessment to maximize growth and development that is age/grade level appropriate.
