Job Summary

Milwaukee Public Schools (MPS) is actively recruiting licensed and experienced candidates for a ;Special Education Itinerant Teacher - Transition Coordinator position. ;

Transition coordinators (TCs) are responsible for acting as a conduit as well as providing services to transition-age youth (14-21) with education disabilities. ; ;Transition coordinators support case managers in creating transition plans for students. ; TCs are the direct liaison between the school-to-Work Program and schools and families.

Transition Coordinators are available to respond to student issues identified at community worksites. ; Community Assessment Training program (CATP) teachers and TCs work to support students regarding assessment, behavior, personal circumstance, etc. ; Transition coordinators are centrally assigned.

Qualifications

Applicants must be licensed by the Wisconsin Department of Public Instruction to teach this specific grade level and / or content area, (801, 810, 811, or 830)

Prefer experience in vocational rehabilitation, job coaching and transition planning

Minimum of 3 years teaching as a fully certified special education teacher or transition coordinator (commensurate agency experience considered on a case-by-case basis).

Minimum of 3 years hands-on experience working with secondary special education students ;or youth in vocational settings.

Ability to provide leadership, demonstrate solid problem-solving skills, and act in a collaborative/ethical manner.

Highly developed written and oral communication skills.

Proficient use of district and program technologies.

Valid Wisconsin Driver's License.

Responsibilities

Trainer ;- ;

Provide staff training in the development of effective Summary of Performances. ;

Develop and give transition presentations on variety of related topics. ;

Assist staff in writing and submitting SOPs. ;

Conduct training in the ;development of transition Individualized Education Plans ;(IEP) that are compliant with Indicator 13. ;

Maintain records and complete paperwork as required. ;

Complete other duties as assigned. ;

Liaison - ; ;

Facilitate and coordinate referral process to CATP, OJT and ETP, visit CATP, OJT and ETP sites, observe ; students¿ performance at sites and communicate results to staff, students and parents.

Create job placements in the community. ;

Maintain communication between post-secondary institutions, employers, employment ;training, and agency providers (e.g. MATC, UW System, DVR, Project Assist, and Milwaukee County Adult Services) during transition process. ;

Work in collaboration with guidance to provide effective transition planning. ;

Facilitate and coordinate visits to post-secondary institutions, visits to career fairs, attendance and participation at Journey Forward Conference and other conferences and assist with job shadowing opportunities. ;

Disseminate transition information to teachers/administrators (including district staff). ;

Resource ¿ ; ;

Attend monthly Transition Coordinator meetings and distribute information from these meetings. ;

Provide consultation services for transition IEPs. ;

Provides information regarding post-secondary institutions, agencies, and other community resources and services related to transition ; ;

Assistance/guidance in accessing transition services as ;well as providing information on ;transition topics ; ;

Promote self-determination activities ; ;

Pay, Benefits, & Work Schedule

The teacher salary range starts at $55,201 and increases according to the set salary schedule based on education beyond the required bachelor's degree and previous years of fully-licensed K-12 teaching experience. ;
