Case Management & Transition PlanningTransition Services: Develop robust Transition Plans for students aged 14+ (or state-specific age), focusing on post-secondary education, employment, and independent living.IEP Compliance: Lead the development of annual IEPs and triennial re-evaluations, ensuring all documentation meets federal and state IDEA standards.Inter-Agency Collaboration: Coordinate with outside agencies (e.g., Vocational Rehab, Department of Developmental Services) to ensure a "warm handoff" after graduation.Advocacy & Leadership: Serve as the bridge between general education teachers, parents, and administration to ensure all accommodations are implemented with fidelity in a departmentalized high school setting.
