Physical Medicine and Rehabilitation Physician roles qualify for EB-2 sponsorship when the position requires a board-certified specialist with a medical degree, or EB-3 for licensed physicians in skilled worker classifications. Verify your ECFMG certification before applying
Most PERM-sponsoring employers require ECFMG certification plus U.S. residency completion before initiating labor certification. Confirm your credentials are fully documented and your medical license is active in the state where the role is located.
Target rehabilitation hospital networks and academic medical centers
Inpatient rehabilitation facilities, academic medical centers, and large hospital systems file PERM petitions far more frequently than private outpatient clinics. Filter your search to employers with established immigration counsel and HR processes for physician sponsorship.
Search sponsoring employers using Migrate Mate
Use Migrate Mate to identify Physical Medicine and Rehabilitation Physician roles at employers with active EB-2 and EB-3 sponsorship history, so you're targeting positions where the PERM process is already familiar to the hiring team.
Benchmark your offer against OFLC prevailing wage levels
PERM requires your offered wage to meet or exceed the DOL prevailing wage for your specific SOC code and geographic area. Use the OFLC Wage Search to confirm your offer qualifies at Level II or above before accepting any position.
Negotiate PERM filing timelines during the offer stage
Ask the employer directly when they plan to initiate labor certification after your start date. Delays are common and PERM can add 18 to 24 months before I-140 filing, so getting a written commitment at the offer stage protects your timeline.
Understand whether EB-2 or EB-3 affects your priority date strategy
If you're from India or China, your priority date wait time differs significantly between EB-2 and EB-3 categories. Review the USCIS Visa Bulletin before your employer files the I-140 to determine which classification gives you the earlier cut-off date.
Green Card Physical Medicine and Rehabilitation Physician: Frequently Asked Questions
Do Physical Medicine and Rehabilitation Physician roles qualify for EB-2 or EB-3 sponsorship?
Both categories apply. EB-2 covers the role when the employer's minimum requirement is an advanced degree, which board-certified physiatrists satisfy through their MD or DO plus residency. EB-3 applies when the position is classified as a skilled worker requiring at least a bachelor's degree or equivalent. Your employer's job description and DOL labor certification filing determine which category is used.
How does PERM green card sponsorship differ from H-1B sponsorship for this role?
H-1B visa provides temporary status in two-to-three-year increments with an annual cap and lottery for most applicants. PERM-based EB-2 and EB-3 sponsorship has no annual lottery, leads to permanent residency, and is initiated by the employer through DOL labor certification followed by an I-140 petition with USCIS. The tradeoff is a longer overall timeline, typically two to four years from PERM filing to green card approval for most nationalities.
Which types of employers sponsor green cards for physiatrists most consistently?
Inpatient rehabilitation hospitals, academic medical centers, VA health systems, and large integrated health networks sponsor EB-2 and EB-3 petitions most frequently for physiatrists. These organizations have established immigration legal teams and structured onboarding processes that accommodate the multi-year PERM timeline. Smaller outpatient practices rarely have the infrastructure to manage the labor certification process. Use Migrate Mate to find Physical Medicine and Rehabilitation Physician roles at employers with verified green card sponsorship history.
What happens to my green card process if I change employers mid-PERM?
Changing employers before your I-140 is approved generally requires restarting the PERM process from the beginning with the new employer. If your I-140 has been approved for more than 180 days, AC21 portability may allow you to transfer the priority date to a new employer in the same or similar occupational category, protecting your place in the queue without refiling PERM.
Can a foreign-trained physiatrist with a non-U.S. residency qualify for PERM sponsorship?
Yes, provided you meet the employer's stated minimum requirements, which typically include ECFMG certification, completion of an ACGME-accredited or equivalent residency program, and an active state medical license. DOL evaluates whether your credentials satisfy the job requirements listed in the PERM application. Some employers require U.S. residency completion as a stated minimum, which would be assessed during the labor certification process rather than by USCIS directly.
