Verify your degree meets specialty occupation
USCIS requires a direct relationship between your degree field and Clinic Director duties. A healthcare administration, nursing, or public health degree works. A general business degree alone may trigger an RFE, so document any clinical coursework separately.
Target federally qualified health centers first
FQHCs and hospital networks sponsored H-1B petitions for management roles even during slow hiring cycles. These employers already have immigration counsel on retainer and are familiar with the LCA filing process DOL requires before your petition moves to USCIS.
Search verified sponsors on Migrate Mate
Filter Clinic Director roles on Migrate Mate by employers with confirmed H-1B LCA filing history for this SOC code. That narrows your list to organizations that have already cleared DOL's prevailing wage certification step for this exact title.
Check prevailing wage before negotiating salary
Use OFLC Wage Search to look up the Level I through Level IV wage for your metro area under the relevant SOC code before you enter salary discussions. Your offer must meet or exceed the certified LCA wage or USCIS will reject the petition.
Request premium processing when your start date is firm
If your employer needs you in the role by a specific date and the regular queue runs long, ask HR to file Form I-907 with USCIS for a 15-business-day adjudication window. Not all employers default to this, so raise it early in the offer stage.
Clarify multi-site management in your support letter
Clinic Directors overseeing multiple locations can face USCIS scrutiny about which worksite address controls the LCA. Your employer's attorney should file an amended LCA or itinerary addendum for each primary site where you'll spend significant time.
H-1B Visa Clinic Director: Frequently Asked Questions
Does a Clinic Director role qualify as an H-1B specialty occupation?
Yes, provided the employer requires at least a bachelor's degree in a specific field such as healthcare administration, nursing, or public health for the position. USCIS evaluates the job duties, not just the title. If the role requires broad business management with no clinical degree requirement stated, the employer may need to document the academic requirement more precisely in the petition.
How do I find employers who sponsor H-1B visas for Clinic Director positions?
Migrate Mate filters Clinic Director openings by employers with verified H-1B LCA filing history under the relevant SOC code, so you can see which organizations have already completed DOL's Labor Condition Application process for this title. This is more reliable than asking during a first-round interview, since not every recruiter knows the company's immigration track record for management-level roles.
Which SOC code applies to Clinic Director for H-1B prevailing wage purposes?
Most Clinic Director roles are classified under SOC 11-9111 (Medical and Health Services Managers) for LCA and prevailing wage purposes. You can confirm the exact classification using O*NET and cross-check wage levels for your metro area using OFLC Wage Search. The wage level assigned affects your negotiated salary floor, so verify before accepting an offer.
Can I transfer an existing H-1B to a new Clinic Director employer?
Yes. Under H-1B portability rules, you can start working for a new employer once they file a non-frivolous H-1B transfer petition with USCIS, as long as you're currently in valid H-1B status. The new employer must still file a certified LCA with DOL before submitting the I-129 petition, so build two to four weeks of lead time into your transition timeline.
What documentation strengthens an H-1B petition for a Clinic Director with an international degree?
A credential evaluation from a NACES-member organization that equates your foreign degree to a U.S. bachelor's or higher is standard. If your degree is in a field adjacent to healthcare management, include a detailed job duties letter showing the direct relationship between that field and the specific responsibilities of the Clinic Director role. USCIS looks for that nexus explicitly.
