Verify your credentials meet specialty occupation requirements
USCIS requires Medical Directors to hold a degree in a directly related field. If you trained abroad, get a credential evaluation through a NACES-member service before approaching employers so licensing delays don't stall your petition.
Target cap-exempt employers first
Hospitals affiliated with universities, nonprofit research centers, and government health agencies can file H-1B petitions year-round without entering the lottery. Prioritizing these employers removes the April registration deadline from your job search timeline.
Search verified H-1B sponsor history on Migrate Mate
Use Migrate Mate to filter Medical Director openings by employers with documented LCA filing history for this role. That history signals the employer has cleared prevailing-wage certification before and knows the process.
Check prevailing wage before negotiating your offer
Your employer must pay at least the DOL prevailing wage for a Medical Director in your work location. Run the OFLC Wage Search using the correct SOC code before signing an offer to confirm the salary meets Level III or IV thresholds.
Ask about concurrent LCA and petition filing timelines
The LCA must be certified by DOL before USCIS receives your I-129. Confirm with HR that they're filing the LCA at least three weeks before your intended start date, especially if you're transferring from a current H-1B status.
Clarify whether your duties trigger a new petition
If a Medical Director role expands your responsibilities into a materially different position than what USCIS approved, your employer must file an amended I-129. Raise this with HR before accepting a promotion to avoid unauthorized work exposure.

Find Medical Director JobsMedical Director H-1B Visa: Frequently Asked Questions
Does a Medical Director role qualify as an H-1B specialty occupation?
Yes. USCIS classifies Medical Director positions as specialty occupations because they require at minimum an MD or DO degree, and typically board certification or graduate-level administrative training. The role must require this specific degree in a directly related field, which physician leadership positions consistently satisfy. Your employer documents this in the LCA and I-129 petition.
Which types of employers sponsor H-1B visas for Medical Directors?
Health systems, academic medical centers, managed care organizations, pharmaceutical companies, and government-contracted healthcare agencies all sponsor H-1B petitions for Medical Directors. Academic medical centers and nonprofit research hospitals have the added advantage of being cap-exempt, meaning they can file outside the annual lottery window. Use Migrate Mate to identify employers with active LCA filing history for this role.
How does the H-1B prevailing wage requirement affect Medical Director offers?
DOL requires your employer to pay at least the prevailing wage for a Medical Director in your specific metropolitan area. For most markets, this falls at a Level III or Level IV wage tier given the supervisory and administrative scope of the role. Your employer certifies this wage in the LCA before filing your I-129, and the certified wage becomes the minimum they're legally required to pay.
Can a Medical Director transfer their H-1B to a new employer?
Yes. H-1B portability lets you start working for a new employer as soon as the new I-129 petition is filed, provided you're maintaining valid status. Your new employer files a fresh LCA and I-129 for the Medical Director role at their organization. If the job duties, worksite, or supervising entity differ materially, the new petition must document those specifics fully.
What happens to H-1B status if a Medical Director role is restructured or eliminated?
If your employer terminates your position, you have a 60-day grace period to find a new sponsor, change status, or depart the U.S. If the role changes significantly, an amended I-129 may be required before you transition. Notify USCIS of any material change in duties, location, or employment terms promptly to avoid a gap in authorized status.
