Verify your ECFMG certification before applying
Internationally trained oncologists must hold valid ECFMG certification to obtain a medical license in most states. Confirm yours is current and matches the name on your passport before any employer begins your H-1B petition, since credential mismatches cause USCIS delays.
Target cap-exempt employers first
Nonprofit hospitals affiliated with universities, federally qualified health centers, and NCI-designated cancer centers are cap-exempt. They can file your H-1B petition at any time of year, so you're not locked into the April lottery registration window.
Use OFLC Wage Search to benchmark oncology prevailing wages
DOL sets prevailing wage by SOC code and geographic area. Run your intended role through OFLC Wage Search before negotiations so you know whether the offered salary meets the Level II or Level III wage required on the Labor Condition Application your employer must certify.
Search verified H-1B sponsors on Migrate Mate
Filter Migrate Mate by Medical Oncologist roles to see employers with confirmed H-1B LCA filing history in oncology. That filing record tells you which institutions have actively sponsored oncologists before, not just which ones say they're open to sponsorship.
Clarify J-1 waiver status before accepting an offer
If you trained on a J-1 exchange visitor visa, a two-year home-residency requirement may bar you from changing to H-1B status until it's fulfilled or waived. Conrad 30, IHS, and appalachian regional commission waivers can resolve this, but the waiver process adds months to your timeline.
Request premium processing once your petition is filed
USCIS premium processing upgrades adjudication to 15 business days. For oncology positions with a credentialing start date tied to hospital board approval, standard processing timelines often miss onboarding windows. Confirm with your employer whether they'll cover the premium processing fee upfront.

Find Medical Oncologist JobsMedical Oncologist H-1B Visa: Frequently Asked Questions
Does a Medical Oncologist role qualify as an H-1B specialty occupation?
Yes. Medical Oncologist positions require at minimum an MD or DO degree, completion of an internal medicine residency, and a hematology-oncology fellowship, which satisfies USCIS's requirement that the role normally requires a bachelor's degree or higher in a specific specialty. The O*NET profile for this occupation confirms the advanced degree standard, and USCIS consistently approves oncology petitions under the specialty occupation definition.
Which types of employers sponsor H-1B visas for oncologists?
Academic medical centers, NCI-designated cancer centers, nonprofit hospital systems, and large multispecialty group practices are the most active H-1B sponsors for oncologists. Many of these are cap-exempt institutions, meaning they can file petitions year-round outside the annual lottery. Community hospitals and private oncology practices can also sponsor but are subject to the 65,000 regular cap and the April registration window. Browse Medical Oncologist roles with confirmed sponsorship history on Migrate Mate.
How does the H-1B cap exemption affect oncology job searches?
Cap-exempt employers, including nonprofit research hospitals and university-affiliated cancer institutes, can file an H-1B petition for you at any point during the year. This matters if you finish fellowship in June or need to start before October 1, when cap-subject approvals take effect. Identifying cap-exempt institutions early in your search gives you more flexible start date options and eliminates lottery uncertainty entirely.
What happens to my H-1B status if my oncology fellowship ends before I have a job offer?
If you're transitioning from a J-1 or F-1 fellowship, your authorized stay has a defined end date. H-1B status doesn't begin automatically. You need an employer to file a petition before your current status expires, or you'll need to leave and re-enter under a different status. Many oncology fellows extend their training or request a short-term clinical research appointment at their fellowship institution to bridge the gap while their H-1B petition is pending.
Can I work at multiple hospital locations on a single H-1B petition as an oncologist?
You can work at multiple locations within the same employer's system, but each worksite requires a separate certified Labor Condition Application on file with DOL before you begin working there. If you're joining a group practice or health system with satellite clinics, confirm with your employer that LCAs cover all sites where you'll see patients. Working at an unlisted location without a certified LCA is a compliance violation that can affect your H-1B status and any future green card sponsorship.
