Orthopedic Physician Assistant roles qualify as H-1B visa specialty occupations because the position requires a master's degree in physician assistant studies. Large orthopedic surgery groups, hospital systems, and ambulatory surgery centers regularly file H-1B petitions for PAs, and the role's consistent demand means sponsoring employers are spread across both urban and rural markets.

Verify your NCCPA certification before applying
USCIS treats PA licensure and national certification as evidence of specialty occupation. Have your NCCPA certificate and state PA license current before an employer files your H-1B petition, or the I-129 is likely to draw an RFE.
Target hospital systems over solo orthopedic practices
Large hospital systems and multi-specialty orthopedic groups have in-house immigration counsel and established H-1B filing workflows. Independent or single-surgeon practices rarely have the infrastructure to manage an H-1B petition without significant friction.
Search LCA filing history on Migrate Mate
Filter by occupation code to see which employers have certified Labor Condition Applications for PA roles. This confirms an employer has already cleared the DOL step and is actively willing to sponsor, not just open to it in theory.
Use OFLC Wage Search to negotiate prevailing wage
Your employer's LCA must certify a wage at or above the DOL prevailing wage for your SOC code and work location. Pull the figure from OFLC Wage Search before your offer negotiation so you know the floor the employer is legally required to meet.
Confirm cap-exempt status if joining an academic medical center
Orthopedic PA positions at nonprofit research hospitals or university-affiliated medical centers often qualify for cap-exempt H-1B status, bypassing the annual lottery entirely. Ask HR whether the institution files cap-exempt petitions before assuming you'll need a lottery slot.
Request premium processing in your offer negotiation
USCIS premium processing reduces adjudication to 15 business days. If your OPT or other status expires before the standard processing window closes, make premium processing a condition of accepting the offer, not an afterthought once the petition is filed.
H-1B Visa Orthopedic Physician Assistant: Frequently Asked Questions
Does an Orthopedic Physician Assistant role qualify as an H-1B specialty occupation?
Yes. USCIS recognizes PA positions as specialty occupations because they require a master's degree in physician assistant studies as a standard entry requirement. Orthopedic PA roles carry the same classification. Your employer will document the degree requirement in the H-1B petition, and your NCCPA certification and state licensure serve as supporting evidence of your qualifications.
Which types of orthopedic employers are most likely to sponsor H-1B visas?
Large hospital systems, academic medical centers, and multi-location orthopedic surgery groups are the most consistent H-1B sponsors for PA roles. They have established immigration processes and legal support in place. Ambulatory surgery centers affiliated with a hospital network also sponsor regularly. Solo or small private orthopedic practices sponsor far less frequently due to administrative complexity. Use Migrate Mate to filter employers with verified LCA filing history for PA roles before applying.
Can I work for an orthopedic employer while my H-1B petition is pending?
If you're transitioning from F-1 OPT, the cap-gap rule extends your work authorization through September 30 of the fiscal year if your OPT expires before October 1 and your H-1B petition was filed on time. If you're already on H-1B with a different employer, you can begin work at the new orthopedic employer as soon as the transfer petition is filed, not after approval.
How does the DOL prevailing wage requirement affect orthopedic PA salaries under H-1B?
Your employer must certify in the LCA that they'll pay you at or above the DOL prevailing wage for your SOC code in the specific metropolitan area where you'll work. Orthopedic PA wages vary significantly by region, so the floor differs by location. You can look up the applicable wage level using OFLC Wage Search before accepting an offer to ensure the employer's figure meets the legal minimum.
Does working at a nonprofit or academic orthopedic center affect my H-1B cap status?
Yes, significantly. If your orthopedic employer is a nonprofit research institution, a university-affiliated hospital, or a government research organization, your position may qualify for cap-exempt H-1B status. Cap-exempt petitions are filed directly with USCIS without entering the annual lottery, and they can be filed and approved at any point during the year. Confirm this classification with the employer's HR or immigration counsel before assuming lottery participation is required.
