Verify your NCCPA certification meets requirements
USCIS requires a directly related degree for specialty occupation status. Your master's-level PA program and active NCCPA certification together establish that credential link. Confirm your state license is current before your employer files the LCA.
Target health systems with dedicated immigration teams
Academic medical centers and hospital networks with in-house legal or HR immigration staff process H-1B petitions faster and with fewer delays than smaller private practices relying on outside counsel for the first time.
Search H-1B filing history on Migrate Mate
Filter by Physician Assistant job titles on Migrate Mate to see which employers have active LCA filings for PA roles. That history signals an employer already understands the process and has filed successfully before.
Clarify supervising physician arrangement before offer
Some states require a formal collaboration or supervision agreement between a PA and a physician. Confirm the employer has that structure documented, since USCIS may scrutinize the employer-employee relationship during adjudication.
Request premium processing if your start date is firm
If you're transitioning from OPT or a cap-exempt position and need an H-1B approval by a specific date, ask your employer to file Form I-907 for premium processing to get a decision within 15 business days.
Use OFLC Wage Search to benchmark your offered wage
Look up the prevailing wage for your SOC code and work location before accepting an offer. Your employer's LCA must certify a wage at or above that level, and a shortfall will stall the filing at the DOL stage.
H-1B Visa Physician Assistant: Frequently Asked Questions
Does a Physician Assistant role qualify as an H-1B specialty occupation?
Yes. PA positions meet the specialty occupation standard because they require at minimum a master's degree in physician assistant studies from an accredited program, plus NCCPA certification. USCIS recognizes the direct relationship between the required degree and the clinical duties of the role, which is the core test for specialty occupation classification.
Which types of employers most commonly sponsor H-1B visas for Physician Assistants?
Academic medical centers, large regional health systems, Veterans Affairs facilities, and multispecialty group practices file the most H-1B LCAs for PA roles. VA facilities are federal entities and cap-exempt, meaning they can file outside the April lottery window. You can browse employers with verified PA filing history on Migrate Mate.
How does the H-1B cap lottery affect PA job seekers?
Most PA employers are cap-subject, so your petition enters the annual lottery with an April 1 filing date and an October 1 start date if selected. Cap-exempt employers, including nonprofit research institutions and VA facilities, can file at any time of year. If you're currently on OPT, your employer must register you in the March lottery to maintain continuous work authorization.
Can a Physician Assistant change employers while on H-1B status?
Yes. Under AC21 portability, you can start working for a new employer once they file an H-1B transfer petition, without waiting for approval, as long as your current H-1B has been approved for at least 180 days. The new employer files a new LCA and I-129 petition covering your new role, location, and wage level.
What role does the DOL prevailing wage play in PA sponsorship?
Before filing an H-1B petition, your employer must obtain a certified LCA from the DOL confirming the offered wage meets or exceeds the prevailing wage for your PA occupation code and work location. You can verify the applicable wage level yourself using the OFLC Wage Search tool. A wage shortfall prevents certification and blocks the entire petition.
