Job Duties:

• Perform comprehensive patient assessments and ongoing reassessments
• Complete OASIS documentation accurately and on time
• Develop individualized plans of care based on patient needs and physician orders
• Provide skilled nursing interventions including wound care, medication management, disease education, and symptom monitoring
• Communicate changes in patient condition promptly to physicians and the care team
• Coordinate care with interdisciplinary team members to support optimal outcomes
• Educate patients and caregivers on disease processes, safety, and home management
• Maintain a safe care environment and adhere to best‑practice standards
• Participate in case conferences and care‑planning discussions
• Assist with transfer and discharge planning
• Recommend referrals to other disciplines or community resources as needed
• Complete timely, accurate documentation in the EHR

Requirements:

• Current Colorado RN license
• Prior home health experience preferred
• Strong clinical assessment and critical‑thinking skills
• Valid driver’s license and reliable transportation
• Proof of auto insurance (MVR required)
• Ability to pass background and national sex‑offender checks

Shift: Full-time & Part Time

Compensation: $35-92 per Visit

Pay: $35.00 - $92.00 per hour
