The Intake LPN is primarily responsible for ensuring timely and accurate processing of home care referrals. Duties include obtaining initial completed referral and general patient information to coordinate the state of care. The Intake LPN coordinates with the patient and/or family/caregiver regarding home care services including collection of demographic data and scheduling preferences. In addition, the Intake LPN provides authorizations for home care services, as applicable, and ensures that scheduling and data entry requirements are consistently accomplished to meet patient needs and state and federal regulations. The position also assists with the general office functions for the agency to include coordinating services and answer patient calls and other duties as assigned.
Primary Responsibilities:
- Accepts calls from referral sources and patients/families/caregivers and obtains accurate and up to date information related to the admission for home care services
- Provides expert customer service in interactions with referral sources and clients
- Obtains required patient information to initiate admission for home care services including, but not limited to, physician orders for home care services, referral information, hospital discharge summary (as applicable) and other documentation
- Assists with telephone coverage for the department and agency and coordination of patient care issues
- Assumes individual responsibility for professional competence and professional nursing practice
- Assists with initial patient visit scheduling if applicable and coordinating services to other disciplines and departments such as Southwest Medical Pharmacy services
- Participates in rotation of office coverage for weekend and holidays
- Promotes positive customer service at all times both internally and externally in interactions with patients, families, referral sources, vendors, co-workers of the department and agency
- Assist with new employee orientation. Attend mandatory meetings and in-services

Required Qualifications:
- High School diploma or equivalent
- Registered LPN with active unrestricted license in the State of Nevada
- 1+ years of experience in home care, community health or medical/surgical patient care setting providing direct patient care
- High knowledge of customer service principles and ability to remain calm and composed in pressured situations
- PC skills with Windows, Outlook or other email systems, Electronic Medical Records systems and experience working with a clinical application system

- Home Care intake processing and scheduling experience

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
