The Role
What You Will Do
- Works with patients and their families to determine goals for treatment.
- Solicits family information, observations and concerns.
- Interviews, observes and develops treatment plans for patients.
- Implements, goals, policies and activities designed to meet program objectives
- Conducts individual and group therapy sessions for patients with mental illness, emotional and substance abuse problems.
- May assess incoming clients and eventually help prepare them for transitioning home.
- Provides referrals to appropriate mental health resources.
- Monitors patient’s treatment progress and keeps track of their mental status.
- Consults with other mental health professionals to provided effective courses of treatment.
- Provides crisis intervention services to patients who are in danger to themselves and others.
- Keeps accurate records of patient records and treatment provided.
- Evaluates and utilizes community mental health resources as needed.
- Keeps abreast of changes in field.
- Performs other duties as requested.
Who You Will Be
- NYSED LMSW or LMHC licensure OR other QHP (Qualified Health Professional) status such as MSW-Limited Permit with ability to obtain within 90 days of hire and licensure within one year of hire or MHC-Limited Permit with ability to obtain within 90 days of hire for designated programs/divisions.
- Master’s Degree in Social Work or Mental Health Counseling.
- At least One-Two years demonstrated clinical experience in substance use and/or mental health treatment for designated programs based on needs of the facility.
- In depth knowledge of NYC diverse network of medical and behavioral health care organizations, including substance use disorders, treatment services, education and employment services, and non-traditional community support groups.
- Computer literacy including proficiency in Microsoft Office Suite and EHR.
- Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
- Ability to maintain confidentiality.
