JOB TITLE: Licensed Therapist (LMSW, Bilingual)
REPORTS TO: CCBHC Program Director
DEPARTMENT: CCBHC
LOCATION: ONSITE
We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This position requires in-person attendance with a team at 1 Brooklyn clinic location with some travel in support of the CCBHC Project.
Certified Community Behavioral Health Clinic (CCBHC) Core Services includes:
- Crisis mental health services including 24-hour mobile crisis teams, emergency crisis intervention, and crisis stabilization
- Screening, assessment, and diagnosis including risk assessment
- Patient-Centered treatment planning or similar processes, including risk assessment and crisis planning
- Outpatient mental health and substance use services
- Outpatient clinic primary care screening and monitoring of key health indicators and health risk
- Targeted case management
- Psychiatric rehabilitation services
- Peer support, counselor services, and family support services
- Intensive community-based mental health care for members of the armed forces & veterans
SUMMARY
Experienced adult mental health bilingual therapist needed for new CCBHC program dedicated to integrating mental health, substance use and medical care. Program will provide care coordination, mobile crisis intervention and ongoing treatment in mental health, medical health and substance use in the Coney Island community. The licensed therapist must conduct intake, evaluation and individual psychotherapy. The person must practice person-centered care, participate in multi-disciplinary team goal setting and reviews, document treatment needs and activities in client record, and continuously assess for risk, including but not limited to suicide. Requirements include attending supervision and training sessions and completing documentation of individual therapy services.
JOB DESCRIPTION:
- Provide bilingual individual therapy to adults with severe and persistent mental illness aimed at identifying and achieving person-centered goals maintaining overall wellness with positive and adaptive coping skills.
- Conduct risk assessments, including but not limited to risk to self and others.
- Provision of services that are OMH compliant and in line with standards of care for CCBHC services.
- Maintain collaborative and compliant documentation that meets OMH and organizational expectations.
- Actively participate on an interdisciplinary team and support CCBHC core service.
- Participate in person-centered treatment planning, including risk assessment and crisis planning.
- Provide DSM diagnostic assessment and treatment plan and make appropriate recommendations and referrals and document all activities in accordance with regulatory agencies policy and procedures.
- Conduct regular collateral follow up with treatment partners as clinically indicated and as directed by your immediate supervisor or the CCBHC Director of Mental Health services.
- Work closely with clients to develop a person-centered treatment plan that addresses mental health co-morbidity treatment, including improving adherence to recommended treatment.
- Evaluate and provide therapeutic interventions in individuals and in groups using evidenced-based practices.
- Complete all paperwork requirements and statistical reporting to ensure program compliance with applicable intergovernmental oversight agencies.
- Maintain accurate and complete records as required.
- Attend all required management and staff meetings and complete all CCBHC and organizational trainings.
Experience:
- 2 years, preferred, clinical experience with assessment and individual therapy
- Experience working with adults
- Experience with peer engagement, preferred
- Knowledgeable regarding the psychosocial aspects of illness and health
- Proficient with word processing, electronic health records, and electronic communications
- Well-organized with strong time management skills
- Enthusiasm and self-motivation regarding identifying client needs and obtaining related resources
- Able to work independently and as a member of a team
- Excellent interpersonal skills, including being able to maintain a professional temperament with both clients and colleagues
- Excellent psychotherapy skills
- Able to meet program standards in documenting in client records
- Ability to work with challenging clients
- Comfortable and competent working with racially, culturally, religiously and ethnically, linguistically, differently abled diverse communities, including but not limited to BIPOC and LGBTQ+ communities
- We need coverage from Sunday through Thursday, including some evening shifts
- Spanish-speaking preferred
QUALIFICATIONS
- Master’s Degree in Social Work (LMSW)
- Licensed in New York, LMSW, LMHC, or Licensed Clinical Social Worker (LCSW)
