Job Summary

Join our Physical Medicine & Rehabilitation (PM&R) practice providing consultative rehabilitation care at two skilled nursing facilities in Salt Lake County.

This is a PM&R consulting role—not primary medical management. The NP/PA works alongside the facility's primary medical team to identify and address medical and functional issues that affect therapy participation, recovery, independence, and quality of life.

The position offers a high degree of clinical autonomy with physician collaboration and support. The schedule is generally 4–5 days per week depending on patient volume, with an expected volume of approximately 75–100 focused PM&R visits per week based on the mix of new evaluations and follow-up visits.

Who We Care For

Our patients are primarily adults receiving skilled nursing and rehabilitation following hospitalization, surgery, stroke, orthopedic injury, neurological illness, or medical decline.

Many have complex functional needs involving pain, weakness and deconditioning, impaired mobility, bowel and bladder dysfunction, sleep disturbance, fatigue, cognitive impairment, mood concerns, and other conditions that can interfere with rehabilitation and quality of life.

What You Will Do

• Perform PM&R consultations, new-patient evaluations, and follow-up visits in two skilled nursing facilities.
• Assess medical and functional barriers to rehabilitation and therapy participation.
• Identify appropriate rehabilitation-related diagnoses based on the patient's clinical and functional presentation.
• Evaluate issues such as pain, bowel and bladder dysfunction, sleep disturbance, fatigue, impaired alertness or cognition, mood concerns, and other quality-of-life factors affecting rehabilitation.
• Develop practical recommendations to improve comfort, therapy participation, functional recovery, and independence.
• Communicate recommendations to the patient's primary medical team rather than assuming responsibility for primary medical management.
• Collaborate with physical and occupational therapists, speech-language pathologists, nursing staff, physicians, and other members of the interdisciplinary rehabilitation team.
• Monitor functional progress and adjust PM&R recommendations as clinically appropriate.
• Complete timely, accurate documentation and communicate significant clinical findings to the appropriate care team.

Schedule & Compensation

• Full-time W-2 position.
• Base salary of $100,000–$125,000, depending on experience, qualifications, patient volume/days worked.
• Additional productivity-based bonus opportunity.
• Typically 4–5 clinical days per week depending on patient volume.
• Approximately 75–100 focused PM&R visits per week, depending on the mix of new evaluations and follow-up visits.
• Practice limited to two skilled nursing facilities in Salt Lake County.

Optional Weekend Income

For clinicians interested in additional income, optional paid weekend coverage may be available within our inpatient rehabilitation facility (IRF) practice.

Weekend work is separate from the full-time SNF position and is not required. Availability and compensation depend on coverage needs.

Qualifications

• Active, unrestricted Utah NP or PA license and current national board certification.
• Strong clinical assessment, medical decision-making, communication, and documentation skills.
• Ability to practice with substantial day-to-day autonomy while collaborating effectively with a rehabilitation physician and facility care teams.
• Experience in PM&R, skilled nursing, inpatient rehabilitation, geriatrics, internal medicine, family medicine, neurology, hospital medicine, or post-acute care preferred.
• Interest in functional recovery, rehabilitation medicine, and improving quality of life for patients with complex medical and functional needs.
