The RN Nurse Navigator – Breast Oncology serves as a clinical resource and patient advocate, guiding individuals and families through the breast cancer care continuum. This role utilizes oncology-specific nursing expertise to provide education, counseling, care coordination, and ongoing support. The Nurse Navigator facilitates timely access to diagnostic, treatment, and supportive services while ensuring patients receive age-appropriate, culturally sensitive, and patient-centered care.
- Full-Time: Monday - Friday 8am - 5pm
- $124,000 - $150,000/year
What you need:
- Bachelor of Science in Nursing (BSN) or equivalent, with a current California Registered Nurse (RN) license and Basic Life Support (BLS) certification from the American Heart Association.
- Minimum of two (2) years of progressively responsible RN experience in an ambulatory or outpatient clinical setting.
- Certified Oncology Nurse (ONC) credential required; Certified Nurse Navigator (CNN) certification preferred.
- Oncology and/or infusion therapy experience with demonstrated knowledge of cancer care, treatment protocols, and patient education strongly preferred.
- Proficiency with Epic EMR or comparable electronic health record systems, with strong documentation, care coordination, and interdisciplinary collaboration skills.
What you will do:
- Coordinated comprehensive patient care by serving as the primary point of contact, facilitating appointments, referrals, treatment planning, and access to diagnostic and supportive services.
- Performed nursing assessments, triage, and telephone counseling while providing evidence-based education on disease processes, treatment options, medications, side effect management, and self-care.
- Collaborated with physicians, advanced practice providers, and multidisciplinary teams to ensure continuity of care, advocate for patients, and support shared decision-making throughout the care continuum.
- Documented patient assessments, care plans, outcomes, and quality metrics in the electronic health record (Epic EMR), maintaining compliance with HIPAA and organizational standards.
- Promoted positive patient outcomes through care coordination, culturally sensitive education, quality improvement initiatives, and connection to community resources and survivorship programs.
