Posting Note: Pediatric Abdominal Transplant Position for Nurse Practitioner or Physician Assistant
Phoenix Children’s is seeking an Advanced Practice Practitioner (APP) to join our growing abdominal transplant team. This position will be one of six APPs providing inpatient and outpatient coverage to the pediatric kidney and liver transplant programs. The Pediatric Kidney Transplantation Program at Phoenix Children's began operations in 2002, and is the only active pediatric kidney transplant program in the state of Arizona. The program has performed over 280 transplants since inception and typically transplants 12-15 patients per year with outcomes meeting or exceeding expected projections. The Pediatric Liver Transplantation Program was launched at Phoenix Children’s in 2012 and is currently performing on average 5 transplants per year with post-transplant outcomes meeting or exceeding expected projections. The liver transplant program currently follows over 150 active patients. This position will collaborate directly with the hepatologists, nephrologists and surgeons providing transplant services, as well as dedicated nutritionist, social worker, and pharmacist. The pediatric liver and kidney transplant programs are a part of the Phoenix Children's Transplant Center which also includes programs in bone marrow and heart transplantation.
Required Qualifications:
- Pediatric Nurse Practitioner or Physician Assistant licensure in Arizona
- At least one year of NP/PA experience in pediatrics or transplant is preferred
- Previous inpatient clinical experience is required, preferably in an environment providing care to transplant patients
General responsibilities include:
The team of six APPs provide inpatient coverage to the liver and kidney transplant programs seven days per week in rotation, and when not providing inpatient coverage provide outpatient coverage to the liver transplant program.
Inpatient work:
- Includes H & Ps, consults, and rounding on patients (primarily in the Pediatric Intensive Care Unit and on the transplant unit), for pre transplant, transplant and post-transplant patients
- Discharge education and coordination of care is extensive in this role
- Requires management of medically complex pediatric patients with end stage liver and kidney disease, as well as post-transplant care including immunosuppressant management
You will be working alongside residents and attending’s as well as many other disciplines within transplant so good communication and interpersonal skills are required.
Schedule: 10 hour days during the week with weekend coverage consisting of 5 hour shifts
This position partners with five other (APPs) to provide 24/7/365 call coverage to the transplant program.
Outpatient work:
- Clinic is located on the Phoenix Children's Hospital campus at 1919 E. Thomas
- Must be able to work autonomously and in partnership with multiple disciplines constantly
- Management of the transplant population including evaluations and consenting for transplant, as well as follow up and coordination of ongoing care
- Patient and family education is extensive in this position
- Supervising and working directly with dedicated transplant Medical Assistants and Registered Nurses
- Active coordination and participation in committee meetings related to transplant patient management
This position works in collaboration with the attending physicians and works within their scope of practice to provide excellent clinical care for patients. Plays a critical role in the patient family medical team. Professional who may practice in hospital’s inpatient and/or outpatient setting.
Position Duties
- Conducts comprehensive health history, physical assessments and plans for infants, children and young adults with acute and chronic health conditions. Utilizes advanced assessment skills to manage a cohort of patients.
- Orders and prescribes pharmacologic and non-pharmacologic interventions, and performs other therapeutic or corrective measures to treat illness and improve health status.
- Formulates family centered plan of care. Performs daily documents as necessary. Collaborated and coordinated care with other team members.
- Collects data and analyzes when necessary.
- Performs any needed advanced procedures that are associated with their credentialing process DOP.
- Utilizes the most up to date and evidence based practice for plan of care and able to alter the plan according to family needs and effectiveness or plan.
- Serves as an educational resource to patient/family/ nursing staff/ provider team and other health care teams as needed.
- Participates in own professional development by maintaining required competencies, certifying body renewals, seeking educational offerings when available, and participates in the professional development of others which may include Advanced Practice Provider students, Residents and Medical Students. Acts as a preceptor/mentor for those new in department.
- Demonstrates commitment to patient safety by bringing up concerns in appropriate manner, reporting through appropriate channels, and ensuring change of practice when necessary. Demonstrates shared accountability of safe practice.
- Performs miscellaneous job related duties as requested.
