Job Title: Nurse Practitioner or Physician Assistant
Schedule: Full-time; 4/10 hour shifts from 7:30-6:00, between Mon-Fri, & occasional Saturdays
Starting Annual Salary: $110,000
Location: 2240 Adams Avenue, Ogden, UT 84401

Nurse Practitioner or Physician Assistant wanted for primary care clinics serving the medically underserved for Midtown Community Health Center. Provide care to uninsured and primarily Spanish-speaking individuals in a community health center setting.
Responsible for the provision of quality health care services. Management of chronically ill patients and the provision of selected interventions according to standards of care and MTCHC policies and protocols. Promotes the maintenance of health through screening, counseling, education and appropriate referrals. Participates in fulfilling MTCHC mission and federal grant requirements by participating in appropriate aspects of the clinical outcomes measure plan, the financial outcomes measure plan, and the strategic plan. Documents provision of care appropriately.
Minimum Qualifications:
Education- Master's Degree from an accredited Family Nurse Practitioner/Physician Assistant program
Experience- 2 years preferred
License required, Utah license preferred
Other Skills:
Ability to communicate in English and Spanish required to meet the mission of MTCHC. Experience with underserved populations preferred.
