Per Diem to start, growing to full-time | Houston, TX (Field-based)
Viking Hospice Care is a locally owned, Medicare-certified hospice agency serving Greater Houston. We're building our clinical team from the ground up and are looking for an experienced hospice RN to serve as our lead clinician — someone who is comfortable working independently and is motivated by long-term growth.
This is a PRN position to start, with hours growing to full-time as our patient census grows.
What You’ll Do
Clinical oversight:
- Manage a direct patient caseload, conducting visits in homes, ALFs, and SNFs across Harris, Fort Bend, Montgomery, and Brazoria Counties
- Facilitate IDT meetings, review Plans of Care, and maintain documentation in compliance with CMS Conditions of Participation
- Conduct clinical supervisory visits with Hospice Aides (at least every 14 days per patient) and audit medical records for quality and completeness
- Manage our QAPI program, HOPE assessments, and overall survey readiness
- Coordinate with the Medical Director on terminal prognosis documentation, CTIs, and physician order management
Day-to-day patient care:
- Perform comprehensive clinical assessments at every visit
- Conduct admission visits including full consent and Plan of Care initiation
- Complete all documentation in Hospice Tools on the day of service
- Perform death pronouncements and manage controlled substance disposal per DEA requirements
After-hours on-call (alternating weeks):
- Triage after-hours calls and provide clinical guidance to patients and families
- Make home visits when clinically indicated; receive additional hourly pay for all time worked including drive time
- Availability during your on-call week is compensated through a flat weekly stipend
What We’re Looking For
Required:
- Current, unrestricted Texas RN license in good standing
- At least 1 year of clinical experience in a Medicare-certified hospice setting
- Solid working knowledge of CMS hospice Conditions of Participation and clinical documentation standards
- Experience with IDT meetings, care planning, and hospice admissions
- Comfortable working independently in the field with limited direct supervision
- Reliable vehicle and willingness to travel throughout Greater Houston
- Smartphone capable of running Hospice Tools (iOS or Android)
Nice to have:
- 2+ years of hospice RN case management experience
- Prior experience in a clinical lead, charge RN, or supervisory role
- Experience with new hospice startup or initial CMS certification
- CHPN certification
- Bilingual in English and Spanish
Compensation
- Based on experience
- After-hours on-call compensated through a flat weekly stipend
- Home visits during on-call periods paid at your full hourly rate including drive time
- Mileage reimbursed at the IRS standard rate
- Hours start at 10–30 per week and grow with patient census
