Job Summary
Elite Prime Care is seeking a Spanish-speaking Medical Assistant to support our Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. This is a fully remote, part-time position (20–30 hours per week) working with patients and healthcare providers to support ongoing chronic care.
The ideal candidate is organized, comfortable communicating with patients by phone, experienced with electronic medical records, and able to accurately document patient interactions. Previous RPM/CCM experience is preferred but not required; training will be provided to the right candidate.
Responsibilities
- Communicate with patients by phone regarding RPM and CCM participation, follow-up, and routine care coordination
- Assist patients with proper use of remote monitoring devices and address basic device-related questions
- Review incoming RPM readings such as blood pressure, pulse, blood glucose, weight, and oxygen saturation
- Contact patients regarding missing readings and encourage consistent device use
- Document patient communications, activities, and relevant information accurately in the EHR and RPM platform
- Escalate abnormal readings or patient concerns to the appropriate clinical team member according to established protocols
- Assist with preparation and maintenance of patient records and care-management documentation
- Support monthly RPM/CCM workflow, patient outreach, and administrative care-coordination activities
- Maintain patient confidentiality and follow HIPAA requirements
- Work collaboratively with physicians, nurses, and care coordinators
Requirements
- Medical Assistant experience or Medical Assistant training/certification
- Fluent Spanish and English required
- Strong verbal communication and patient-service skills
- Comfortable making multiple patient calls throughout the workday
- Experience using an EHR/EMR or other healthcare documentation system
- Basic understanding of medical terminology and vital signs
- Strong computer skills and ability to learn new healthcare software
- Excellent attention to detail and documentation skills
- Ability to work independently in a remote environment
- Reliable computer, internet connection, and appropriate private workspace for handling confidential patient information
Preferred Qualifications
- Previous experience with Remote Patient Monitoring (RPM), Chronic Care Management (CCM), care coordination, primary care, or chronic disease management
- Experience communicating with Medicare and/or elderly patients
- Familiarity with hypertension, diabetes, and congestive heart failure
- Experience documenting patient interactions in an EHR
- Familiarity with HIPAA requirements
