The transitional care research associate is a core member of the Transitional Care Excellence team supporting patients with complex cardiovascular, renal, metabolic, and pulmonary conditions during critical transitions from hospital to home. This role works directly with patients, caregivers, and a multidisciplinary clinical team to ensure safe, supported transitions of care, reduce avoidable hospitalizations, and improve long-term disease outcomes.
Responsibilities
Participates in the design, conduct, and evaluation of implementation research and cohort studies focusing on transitional care interventions and their associated health outcomes
Coordinates follow-up appointments with primary care and subspecialty teams, including cardiology, nephrology, and pulmonology
Follows an assigned cohort and outreaches patients telephonically to ensure effectiveness of interventions
Collaborates with TCE clinical team to support implementation of care plans and therapies
Identifies and escalates clinical and social risks, including worsening symptoms, medication access issues, and unmet social needs, to the TCE clinical team
Assists with implementation of departmental transitional care protocols and pathways
Supports patients in navigating the healthcare system, including use of patient portals, telemedicine visits, and remote monitoring tools
Collects qualitative and quantitative data from multiple sources including patients, caregivers, clinicians, and electronic health records
Analyzes and interprets data in Excel, Power BI or other software
Requirements
Bachelor’s degree required, life sciences or public health background preferred
MS Office proficient, Power BI or Tableau experience preferred
Ability to manage multiple tasks and priorities with exceptional attention to detail
Bilingual/Spanish-speaking preferred
Evidence of excellent interpersonal and communication skills, creative problem solving, critical thinking and leadership
Previously demonstrated ability to facilitate group processes and work cohesively and collaboratively as member of the interdisciplinary team
Qualified candidates must be able to effectively communicate with all levels of the organization
