Description:
The Case Manager LMSW is a professional who collaborates with physicians, health care team members and patient/families to coordinate care for specific patient populations across the continuum and between care settings during an episode of illness. Job Status: Casual

Position Purpose:
Case Manager LMSW I to work in the Case Management Department at Grand Haven Hospital.
What you will need:
- Masters degree in social work required
- Prior employment experience in a related area preferred
- State of Michigan, social work license required
What you will do:
- Identify, coordinate communicate and participate in discharge planning with patients and a multidisciplinary team
- Recognize and monitor potential barriers to safe discharge planning and intervene as appropriate
- Navigate community resources as well as serve as a reference and liaison for community resources and service
- Perform conflict resolution and crisis intervention when and where appropriate
- Accurately document interventions, assessments, and discharge planning in the electronic medical record
