Summary:
The PHSO Quality Improvement Specialist is responsible for helping achieve quality performance in value based care contracts. The role involves analyzing healthcare gaps, improving patient engagement, and educating on the need to perform preventative healthcare screenings. They are responsible for documenting in electronic medical records (EMR), payor portals, or other required systems to ensure accurate and complete records. They will perform low risk outreach to patients discharged from an acute setting and refer to Care Coordinators or providers when appropriate. The Quality Improvement Specialist will collaborate with the health care team to achieve quality performance outcomes in value based care contracts.
BASIC QUALIFICATIONS
- Licensed Practical Nurse, Medical Assistant Certification, or relevant experience as a Pharmacy Technician in the state of AR is required.
- 2 years or more experience in ambulatory healthcare or pharmacy setting is required.
- AR Driver's License
This job will be authorized 80.00 hours bi-weekly.
