The Early Onset Breast Cancer - Program Coordinator supports the development and sustainability of the Early Onset Breast Cancer Program in alignment with the contractual agreement through CT DPH grant funding. Responsibilities include administrative tasks such as maintaining databases; collecting and organizing data; managing program registries, quality metrics, and reports; and ensuring compliance with grant requirements.
The Program Coordinator also provides direct patient support, including conducting comprehensive biopsychosocial assessments and identifying appropriate internal and external resources to address long-term needs. Additional expectations include developing programs that promote system-wide, comprehensive support focused on patient care, staff education, and community outreach, in alignment with the Cancer Institute’s mission to deliver whole-person, patient-centered care.
Qualifications
Education:
- Bachelor’s degree in a healthcare related field (Social Work, Psychology, Nursing, Health Science) required
Experience:
- Minimum of two (2) years of experience providing support services to cancer patients within a Cancer Center required
- EPIC preferred
- Grant management experience preferred
