Position Summary
The IDD Program Specialist supports individuals with Intellectual and Developmental Disabilities (IDD) by overseeing and optimizing program services, ensuring compliance with state and federal regulations, and fostering a person-centered approach to care. This role collaborates with interdisciplinary teams to promote quality outcomes and enhance community inclusion.
Key Responsibilities
Program Coordination:
- Develop, implement, and monitor Individualized Service Plans (ISPs) in accordance with person-centered care principles.
- Ensure all services align with participant goals while meeting state and federal standards.
Case Management:
- Conduct assessments and coordinate services tailored to individual needs.
- Advocate for participants to secure necessary supports and resources.
- Manage a caseload of participants totaling a minimum of 800 billable weekly hours, while supervising all Direct Support Staff.
- Conduct home visits and support participants in achieving ISP-defined goals for measurable, positive outcomes.
Compliance and Documentation:
- Maintain accurate and timely documentation, including progress notes, incident reports, and evaluation summaries.
- Ensure all records meet compliance and quality assurance requirements.
Staff Training and Support:
- Train and guide Caregivers on best practices, policies, and individualized care plans.
- Provide continuous support, supervision, and professional development opportunities for staff.
Community Engagement:
- Facilitate community resource connections to foster integration and inclusion.
- Plan and coordinate social and skill-building activities for participants.
- Attend and contribute to monthly team meetings, ISP meetings, and Annual Review Updates as required.
Quality Assurance:
- Monitor service delivery for quality and effectiveness, implementing program improvements as necessary.
- Participate in internal audits and regulatory reviews to ensure compliance and enhance service standards.
Qualifications
Education
Bachelor’s degree in Social Work, Psychology, Special Education, or a related human services field.
Experience
Minimum 2 years of experience in IDD services or a comparable role.
Familiarity with California IDD systems and regulations preferred.
Skills
- Strong understanding of person-centered planning and positive behavior support strategies.
- Excellent organizational, communication, and problem-solving abilities.
- Proficiency in case management systems and electronic documentation.
Certifications and Requirements
- Minimum age: 18 years
- CPR/First Aid certification (preferred)
- Valid California Driver’s License and reliable transportation
