This position is located within the Child and Family Care Coordination (CFCC) unit and works with youth with more intensive behavioral health treatment needs and their families. This position focuses on engaging families by assisting them in developing a plan of care for their child(ren); identifying, referring, and linking to appropriate behavioral health treatment; leveraging natural and community supports; coordinating services, monitoring progress, and providing updates to other systems including Court when applicable. The work hours may occasionally extend beyond the traditional 8:00am - 4:30pm workday.
Essential Functions
Essential functions are fundamental, core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class. - Participates in treatment planning and discharge planning in conjunction with the facility personnel as well as with members of other involved agencies to ensure that the treatment results in the expeditious release of the adolescent patient.
- Designs the aftercare treatment plans and refers clients to appropriate outpatient treatment facilities, community support systems, and/or residential treatment facilities and ensures that the discharge recommendations and aftercare plans are carried out.
- Interacts with clients' families interviewing members, observing family dynamics, assessing strengths or dysfunctions and adapts aftercare treatment plan to reflect their needs as well as those of the client.
- Provides direct supportive services including home visits, individual and family counseling as well as family crisis intervention.
- Monitors the outpatient treatment components to ensure progress and resolve any problems that might arise.
- Develops and maintains a "bank" of community resources to provide the necessary services for the adolescent and family.
- Prepares monthly statistical, narrative and court reports, as required detailing caseload activities.
- Prepares progress notes on each client. Participates in staff meetings.
- Makes formal presentations before community groups, schools and/or professional organizations.
JOB REQUIREMENTS for Adolescent Treatment Services Coordinator
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:
- Possession of a Bachelors degree or higher in Behavioral or Social Science or related field.
- Six months experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
- Six months experience in making recommendations as part of a client's service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
- Six months experience in interpreting laws, rules, regulations, standards, policies, and procedures.
- Six months experience in narrative report writing.
- Possession of a valid Driver's License (not suspended, revoked or cancelled, or disqualified from driving).

This position is a classification organized under an exclusive bargaining representative (labor organization) that has been elected by employees as their representative for collective bargaining and other applicable terms and conditions of employment, in accordance with Title 29, Chapter 59 and Title 19, Chapter 13 and 16. This position is covered by a collective bargaining agreement based on its individual terms.

Criminal background check: A satisfactory criminal background check is required as a condition of hire. The recruiting agency may require the applicant to pay for the criminal background check as part of the conditional offer of hire.
All new hires are required to report to the Statewide New Employee Orientation (SNEO) on their first day of employment. Sessions are held at designated locations in Kent or New Castle County.
