Be the Best Hospice Nurse Practitioner You Can Be
If you meet these qualifications, we would love to meet you:
- Graduated from an approved school of professional nursing - Masters degree
- Current and unencumbered advanced practice nurse license in the state(s) practicing
- One year of experience as an NP
- Certified by a nationally recognized advanced nurse practice organization
- Possess and maintain valid CPR certification while employed in a clinical role (preferred)
- Must be a licensed driver who can travel to all business locations
- Meet the regulations and requirements of the state(s) in which the program provides services
Responsibilities:
As a Hospice Nurse Practitioner, you will:
- Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family
- Monitor patient progress; update the Team Director and team members; consult regarding discharge planning and patient/family teaching
- Maintain continuity of care in coordinating with the care team and case manager for patients with ongoing needs
- Prescribe medication as permitted by the state and/or the state Board of Nursing
Pre-Hospice
- Face-to-face clinical and needs assessment at intake, preferably in the patient's home setting, inclusive of patient's medical, emotional, spiritual, social, and practical needs
- Face-to-face reassessment done when needed and at least every 60 days. Plan of care development based on these assessments and incorporating the patient's and family's priorities
- Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family
- Clinical supervision of the care plan including all medicine orders
- Collaborate with the rest of the pre-hospice team including alerting the team to changes and newly identified patient needs as they arise
