Position Summary
We are seeking a detail-oriented and knowledgeable MDS Coordinator to oversee the completion and accuracy of the Minimum Data Set (MDS) process and support quality clinical outcomes. This role is responsible for coordinating interdisciplinary assessments, ensuring regulatory compliance, and optimizing reimbursement while maintaining a strong focus on resident-centered care.
KEY RESPONSIBILITIES:
- Coordinate and complete MDS assessments in accordance with federal and state regulations
- Ensure accuracy and timely submission of MDS and supporting documentation
- Manage RAI process and care area assessments (CAAs)
- Collaborate with interdisciplinary team to develop and update care plans
- Monitor case mix index (CMI) and identify opportunities for optimization
- Ensure compliance with Medicare, Medicaid, and payer requirements
- Participate in quality assurance and performance improvement initiatives
- Assist with survey readiness and regulatory audits
- Educate nursing staff on documentation requirements
- Review clinical documentation to support appropriate coding and reimbursement
QUALIFICATIONS
- Active RN or LPN license in good standing (state-specific)
- Previous MDS experience in long-term care or skilled nursing preferred
- Strong knowledge of RAI guidelines and reimbursement systems (PDPM)
- Detail-oriented with strong organizational skills
- Excellent communication and interdisciplinary collaboration abilities
- Experience with electronic medical records preferred
SCHEDULE & COMPENSATION
- Full-time position: 40 hours per week + on call
