INTRODUCTION
This position is Weekend: Saturday - Monday. Saturday/Sunday start at 5:30am (12 hour shifts) and Monday is 8 hr shift.
ROLE AND RESPONSIBILITIES

Serves as a team member and performs basic nursing process skills independently. Seeks collegial consultation and plans for care across the continuum for acute patients of hospitalists.

Provides patient care services utilizing the nursing process.

Moves the team toward quality outcomes for patient population.

Develops, monitors and trends performance improvement information and then plans for changes as needed.

Performs care planning activities utilizing input from all team members encompassing individual patient needs throughout the continuum of care.

Provides assessment, planning, implementation and evaluation activities for patient population.

Serves as a resource for client/patient/staff/family and community.
-
Acts as a resource and support for staff in managing cardiac patients for actual care needs as well as planning education and discharge needs.

Identifies staff/family/patient/community needs and implements plans developed.
-
Communicates patient/client/family health care needs to the appropriate health care provider.

Interacts with all shifts, patients and families.
-
Delegates and communicates patient care to appropriate team members based on skill, knowledge, and patient acuity.

Facilitates patient education and discharge planning utilizing interdisciplinary resources.
BASIC QUALIFICATIONS

Education: Associate's Degree Graduation from an accredited, state-approved school of nursing

Licensure: Current MS RN, compact license

Experience: 2 years caring for acute care population

Skills: Strong communication skills

Preferred Education: Bachelor of Science in Nursing (BSN)

Preferred Experience: 4 years caring for acute care population
